Divided over six chapters, "Grandma" tells the story of Elle, a lesbian poet coping with the recent death of her long-term life partner. She ends a four-month relationship with a younger admirer, Olivia, before receiving a visit from her 18-year-old granddaughter Sage, who is pregnant and requests $630 for an abortion. As Elle is broke and Sage has had her credit card confiscated by her overbearing mother, the two embark on a road trip to come to terms with their troubles. They visit the father of the child, who is unable to come up with much money. They then visit Elle's ex-husband Karl, who Elle has not seen in years. Elle tells him she needs to borrow the money for rent, and he requests a kiss in return. After an argument over how Elle left the relationship, he agrees to hand over the cash, then refuses when he learns Sage will have an abortion with it.

In 1991, four childhood friends reunite in their hometown of Shelby, Indiana. Samantha Albertson (Demi Moore), a science-fiction writer, narrates the story. Child psychologist Samuel Faulkner (Hugh Grant) has an ideal romance with ballet teacher Rebecca Taylor (Julianne Moore). Rebecca is thinking about marriage and children. Samuel is against the idea of marriage as he is happy with how things are between them. This all changes when Rebecca declares she is pregnant and when questioned by Samuel about her birth control she replies birth control is only 97% time effective. His fears mount due to his encounters with overbearing couple Marty (Tom Arnold) & Gail (Joan Cusack) and their unruly daughters, as well as the confusing advice he gets from his perpetually single artist friend (Jeff Goldblum). They meet their Doctor Kosevich (Robin Williams) who happens to be Russian. Samuel is confused and unsure about what to do. Feeling neglected Rebecca leaves him and moves in with Marty and Gail. Samuel tries to contact her but she does not respond. When a girl makes a move on Samuel he declines saying he's not ready to move on yet. When he sees an ultrasound of his soon-to-be-born son, he decides that it is time to take responsibility before it is too late. He sells his Porsche buys a family van and gets back together with Rebecca. He marries her and they have the baby together.

Brad Hamilton is a popular senior who is looking forward to his last year of school and almost has his 1960 Buick LeSabre paid off. He has a part-time job at a burger joint, "All American Burger" where his girlfriend, Lisa, also works. This esteemed establishment has a strict policy of etiquette: "No Shirt, No Shoes, No Dice". He is pondering how to end his relationship with Lisa so he can play the field during his senior year. However, Brad is fired for losing his temper at an obnoxious customer; when he tries to tell Lisa how much he needs her, she says she is dumping him to see other guys. Brad quits his next job at Captain Hook Fish & Chips because of the humiliation of having to wear a pirate costume when delivering food. He gets a job at a convenience store, Mi-T-Mart, where he successfully thwarts an attempted robbery and is promoted to store manager.
