In the woods outside of Cherry Falls, Virginia, a teenage couple, Rod Harper (Jesse Bradford) and Stacy Twelfmann (Bre Blair) are getting romantic in a car when a black-haired female appears and murders them both. Meanwhile, in town, teenager Jody Marken (Brittany Murphy), the daughter of the local sheriff, is with her boyfriend, Kenny (Gabriel Mann), who thinks it is time to go "see other people." Jody goes back home to find her father, Brent (Michael Biehn), upset that she is out past her curfew. Brent and his deputies begin to investigate the murders the next day. They see that the killer carved the word "virgin" into both victims. At school, Brent sees English teacher Mr. Marliston (Jay Mohr), who urges him to divulge more details of the murder to students and the town so as to eliminate the possibility of secrets.

As nuclear explosions ravage New York City, residents of an apartment block rush downstairs to escape from the building, only to be forced into the basement by further explosions. Only eight of the residents – Eva (Lauren German) and her boyfriend Sam (Iván González), Josh (Milo Ventimiglia) and his brother Adrien (Ashton Holmes), Josh's friend Bobby (Michael Eklund), Marilyn (Rosanna Arquette) and her daughter Wendi (Abbey Thickson), and Devlin (Courtney B. Vance) – manage to force their way into the building's bomb shelter before the superintendent, Mickey (Michael Biehn), seals the door. The group acclimatises to the cramped surroundings while Mickey asserts his dominance over the shelter and its denizens, much to the chagrin of Josh, Bobby, and Devlin. After some time, the shelter's door is broken open and the shelter is invaded by armed soldiers in biohazard suits; the men's speech is unintelligible and their allegiance remains ambiguous. The men attack the group and seize Wendi, leaving the shelter with her. Devlin and Mickey manage to kill three of the men and Mickey takes a rifle. Josh volunteers to use one of the dead soldier's suits to leave the shelter and search for Wendi.

Many years before the events in The Exorcist, the young Father Lankester Merrin (played by Skarsgård, who played the same part in Exorcist: The Beginning) travels to East Africa. Two men, Brad and Ryan, wake up in a metropolitan storefront window. Each man is bound at the wrists to opposite side of a worktable with a circular saw in front of them. Their mutual lover, Dina, is suspended above a third saw. As a crowd gathers and police attempt to break the window, a puppet informs the men that one of them can kill the other or they can let Dina die. They initially fight each other, during which Brad is cut, but they soon realize Dina's betrayal and allow her to die instead.

The film opens up with surveillance camera views of a mansion and its rooms; the views alternate among images of nine people being kidnapped. Lea (Kelly Brook), an abductee, wakes up in a bedroom.
