Psycho IV: The Beginning is a 1990 made-for-cable-television film sequel and prequel to Alfred Hitchcock's 1960 film Psycho, as it includes both events after Psycho III while focusing on flashbacks of events that took place prior to the original film. It is the fourth and final film in the Psycho series. Directed by Mick Garris, it starred Anthony Perkins, Henry Thomas, Olivia Hussey and CCH Pounder and premiered on the Showtime cable network on November 10, 1990.

The film was written by Joseph Stefano, who also wrote the screenplay of the original film. The original musical score was composed by Graeme Revell and the title theme music by Bernard Herrmann from the original film was used.

Synopsis

Norman Bates is released from the mental hospital again, after having been re-incarcerated at the end of Psycho III; after spending several years there, he is judged rehabilitated for the second time. Norman is now married to a young nurse named Connie and is expecting a child. Norman secretly fears that the child will inherit his mental illness, so he must seek closure once and for all.

At a government laboratory in rural California, a weaponized version of influenza (called Project Blue) is accidentally released, immediately wiping out everyone on staff except for military policeman Charles Campion and his family, who flee the base. However, Campion is already infected by the superflu, nicknamed "Captain Trips", and spreads it to the outside world. That evening, Campion crashes his car at a gas station in East Texas where Stu Redman (Gary Sinise) and some friends have gathered. When they investigate, they find Campion dying of the flu next to his wife and baby daughter, who are already dead. Campion tells Stu with his dying breath that he was followed from the base by a mysterious figure, and states "You can't outrun the Dark Man". The next day, the U.S. military arrive to quarantine the town. While the other townspeople quickly become ill and die, Stu remains healthy and is confined at a CDC facility in Vermont order to study a possible cure. This proves futile and the superflu rages unchecked, causing civilization to collapse and killing over 99% of the population of the entire world in less than two months.

During a lunchtime tryst in Phoenix, Arizona, a real estate secretary named Marion Crane discusses with her boyfriend, Sam Loomis, how they cannot afford to get married because of Sam's debts. After lunch, Marion returns to work, where a client drops off a $40,000 cash payment on a property. Her boss asks her to deposit the money in the bank, and she asks if she can take the rest of the afternoon off. Returning home, she begins to pack for an unplanned trip, deciding to steal the money and give it to Sam in Fairvale, California. She is seen by her boss on her way out of town, which makes her nervous. During the trip, she pulls over on the side of the road and falls asleep, only to be awakened by a state patrol officer. He is suspicious about her nervous behavior but allows her to drive on. Shaken by the encounter, Marion stops at an automobile dealership and trades in her Ford Mainline, with its Arizona license plates, for a Ford Custom 300 that has California tags. Her transaction is all for naught - the highway patrolman sees her at the car dealership and witnesses her purchase of the newer car.

In the Nevada desert, a couple, Peter and Mary Jackson are driving just as they are stopped by a sheriff, Collie Entragian. He soon learns they are in possession of marijuana. In response, he takes them to jail. After they enter the police station, they see a little girl, dead, on the floor and Entragian shoots and kills Peter. Mary is thrown in a jail cell along with a young boy, David Carver, his parents, Ralph and Ellen, and an old man, Tom Billingsley. It's also learned the little girl is the Carver's daughter named Pie. In jail. David is praying to God, which he's been doing since his best friend Brian was struck by a drunk driver. In a flashback sequence, David and Brian are bike riding when a drunk driver strikes Brian, the impact causing Brian to fly through the air and into a building. Brian has suffered severe head trauma and injuries to his face. David immediately begins praying to God, offering to sacrifice anything, and to do what ever is asked of him in order to save Brian. At which point, Brian, who was on the brink of death, miraculously regains consciousness. The doctor at the hospital describes Brian's recovery as "miraculous".
