The movie opens with the feral Woman (Pollyanna McIntosh) circling what appears to be her child. A wolf, apparently tamed by the feral Woman, circles the infant as well but does it no harm. Although it is not referenced in the film, the Woman is the last remaining member of a cannibalistic tribe that has roamed the north-east coast for decades (as seen in the 2009 film Offspring)., 1h23

John (Scott Adkins) awakens from a coma to find that his wife and daughter were brutally murdered in a home invasion. Haunted by images of the attack, he vows to kill the man responsible: Luc Deveraux (Jean-Claude Van Damme), one of the original Universal Soldiers (UniSol). While John tries to piece his reality back together, things are complicated by a relentless UniSol named Magnus (Andrei "The Pit Bull" Arlovski), who is determined to kill John. Meanwhile, Deveraux and surviving UniSol Andrew Scott (Dolph Lundgren) are preparing to unleash anarchy and build a new order ruled by Unisols. They are weeding out the weak and constantly testing their strongest warriors in brutal, life-and-death combat. Luc has emerged operating the Unisol Church of Eventualism, taking in wayward Unisols whom the government has been secretly operating as remote controlled sleeper agents. His mission is to liberate these Unisols from the implanted memories and the lies the government has inserted in them. At a run-down gas station in the middle of the desert, an old man, Fred (John Steadman), is packing his truck when a ragged and somewhat feral teenage girl named Ruby (Janus Blythe) approaches him. She offers to trade what she has in her bag for food, but the old man refuses. They walk into a small cabin, and Fred scolds her for what "they" have done. Ruby says that her family ambushed a nearby airfield because they were hungry and no one passes by their home anymore. She pleads with Fred to take her with him, but he refuses and warns Ruby that she could be in danger if "the pack", in particular someone named Jupiter, learns what she is doing. She replies that his life will be in danger as well if Jupiter finds out that he is trying to leave. A noise distracts them, and Ruby hides.
