In 1976, eight-year-old Mary Daisy Dinkle (Bethany Whitmore) lives a lonely life in Mount Waverley, Australia. At school, she is teased by her classmates because of an unfortunate birthmark on her forehead; while at home, her distant father, Noel, and alcoholic, kleptomaniac mother, Vera, provide little support. Her only comforts are her pet rooster, Ethel; her favourite food, sweetened condensed milk; and a Smurfs-like cartoon show called The Noblets. One day, while at the post office with her mother, Mary spots a New York City telephone book and, becoming curious about Americans, decides to write to one. A suicidal Hunter "Patch" Adams (Robin Williams) commits himself into a mental institution. Once there, he finds that using humor to help his fellow patients gives him a purpose in life. Because of this he wants to become a medical doctor and two years later enrolls at the Medical College of Virginia (now known as VCU School of Medicine) as the oldest first year student. He questions the school's soulless approach to medical care and clashes with the school's Dean Walcott (Bob Gunton), who believes that doctors must treat patients as patients and not bond with them as people. Because of this and incidents such as setting up a giant pair of legs during an obstetric conference, he is expelled from the medical school, although he is later reinstated due to his methods actually helping patients improve. Adams encourages medical students to work closely with nurses, learn interviewing skills early, and argues that death should be treated with dignity and sometimes even humor.

Larry Gigli (Ben Affleck) is a low-ranking Los Angeles mobster who isn't nearly as tough as he likes to act. In New York City in 1975, after serving only five years of a 30-year prison sentence, Carlito Brigante is freed on a legal technicality exploited by his lawyer, Dave Kleinfeld, infuriating the district attorney. Brigante returns to his old neighborhood of Spanish Harlem, where he reconnects with old associates. Although he vows that he is finished with crime, Brigante is persuaded to accompany his cousin Guajiro to a drug deal at a bar. Guajiro is betrayed and killed while Brigante is forced to shoot his way out. He takes Guajiro's money and uses it to buy into a nightclub, with the intent of saving $75,000 to retire to the Caribbean.
