An arrogant wealthy capitalist named Raffaella (Mariangela Melato) is vacationing on a yacht in the Mediterranean Sea with friends—swimming, sunbathing, and talking incessantly about the virtues of her class and the worthlessness of the political left. Her nonstop political monologue infuriates one of the underclass deckhands on her yacht, Gennarino (Giancarlo Giannini), a dedicated Communist who manages to restrain his opinions to avoid upsetting his boss and losing his good job. Despite her humiliating insults, Gennarino agrees to take her out on a dinghy late in the evening to see the rest of her friends who have gone ahead without her. On their way, the outboard motor gives out, leaving them stranded in the middle of the sea with no land in sight.

The film presents the tale of Agnese Ascalone, daughter of prominent miner Vincenzo Ascalone, and takes place in a small town in Sicily, as did Germi's previous film Divorce, Italian Style. Agnese is seduced by her sister Matilde's fiance, and has a tryst with him for which she confesses and tries to repent, only to be discovered by her mother and father. Vincenzo immediately demands that the man, Peppino Califano, marry his daughter, and antics ensue. The film is a dark satire of Sicilian social customs and honor laws, and is very similar to Divorce, Italian Style.

Frank Castle is the city's most wanted, and most mysterious, vigilante, known as "The Punisher". He has killed 125 criminals in the past five years. This combined with Castle's furtive methods of attack have made the Punisher's identity a mystery to the general public. Living in the sewers and waging a one-man war against organized crime, his only friend is an old alcoholic named Shake, a former stage actor who speaks in rhyme for reasons unexplained. Now legally declared dead, Castle strikes back from beyond the grave, killing mobsters wherever he can find them. Due to his war against them, the Mafia families have weakened, forcing one of the family leaders, Gianni Franco, to come out of retirement. Franco has a plan to unify the families. This, however, has attracted the attention of the Yakuza, Asia's most powerful crime syndicate. Led by Lady Tanaka, the Yakuza decide to take over the Mafia families and all of their interests. Après deux années passées sur le continent, Salvatore retourne dans sa Sicile natale. Alors qu'il rêve d'être accueilli en fils prodigue, il constate que les habitants de son village l'attendent, plutôt impatiemment, afin d'y dénouer un conflit d'ordre social. La question cruciale est celle-ci : faut-il occuper les domaines que la récente loi a expropriés et partagés et que la Mafia empêche qu'on leur distribue ? Les paysans entament une action pacifique, mais Salvatore veut aller plus loin : en labourant et semant, ils doivent provoquer une situation irréversible. Les paysans se mettent au travail, mais la Mafia les attaque à l'aube. Les carabiniers interviennent alors pour empêcher les combats. Ne s'avouant pas vaincue, la Mafia entreprend de contrôler l'affermage des carrières du voisinage. Simultanément, elle cherche à gagner à sa cause Salvatore, en l'achetant si nécessaire. Bien qu'il feigne d'accepter la négociation, Salvatore engage une campagne pour améliorer les conditions de travail dans les carrières. Un matin, Salvatore s'en prend violemment au chef local de la Mafia : celle-ci le liquide physiquement, une heure après. Le village entier, lui rendant hommage, assiste à ses funérailles.

Sicile, 1961. L'officier Bellodi, des Carabiniers doit mener une enquête sur le meurtre de Salvatore Colasberna, un chef d'entreprise de bâtiments assassiné pour avoir refusé de confier un chantier à une entreprise protégée par la mafia. Le capitaine Bellodi parvient à arracher à Rosa l'aveu que son mari lui a confié avoir vu le long de la route, le jour du crime, un certain « Zecchinetta », nom qui est confirmé par un informateur de la police, « Parrinieddu ». Entre-temps, le boss (chef, capo en italien) local de la mafia, Don Mariano Arena, cherche à déguiser le meurtre de Colasberna en crime passionnel et fait remonter les pistes vers Nicolosi : il a tué le chef d'entreprise en tant qu'amant de sa femme. Le capitaine ne croit pas à cette version et cherche le corps de Nicolosi, dont il pense qu'il a été tué comme témoin gênant ; de corps, il ne trouvera que celui de « Parrinieddu », tué parce que compromis. L'enquête aboutit à l'arrestation de Don Mariano mais, grâce à ses accointances politiques, il est libéré et Bellodi est transféré.

The film opens in 1900 as Roberto Colombo, an Italian-American attorney living in the Little Italy section of New York City, is killed by gangsters as he meets with a police officer to give information about an attempt to extort money from him.
