Famed novelist Paul Sheldon (James Caan) is the author of a successful series of Regency romance novels featuring a character named Misery Chastain. Wanting to focus on more serious stories, he writes a manuscript for the final Misery novel. While traveling from Silver Creek, Colorado to his home in New York City, he is caught in a blizzard and his car goes off the road, rendering him unconscious. Paul is rescued by a nurse named Annie Wilkes (Kathy Bates), who brings him to her remote home. When Paul regains consciousness he finds himself bedridden, with both his legs broken as well as a dislocated shoulder. Annie claims she is his "number one fan" and talks a lot about him and his novels. As a reward for saving him, Paul gives Annie his new manuscript which she saved from the wreckage. While feeding him, she is angered and spills soup on him but regains control and apologizes. She buys a copy of Paul's most recently published book, Misery's Child, giving glowing praise to Paul as she progresses through the book. However, when Annie discovers that Misery dies at the end of the book she flies into a rage, almost smashing a table on Paul's head. She reveals that she lied about calling his agent and the authorities; nobody knows where he is. Annie leaves and Paul tries to escape from his room, but she has locked the door.

Berlin techno DJ and producer Martin Karow (nicknamed Ickarus) is touring the techno clubs of the world with his girlfriend Mathilde while working on a new studio album that he plans to release soon. In order to be able to work and party day and night, Ickarus takes all kinds of drugs, mainly supplied by his friend Erbse at the clubs in Berlin. After consuming a PMA-containing ecstasy tablet, Ickarus goes into a drug-induced psychosis, eventually finding himself naked in a Berlin hotel where his antics attract the attention of the hotel staff. When an old man (Sebastian Cabot) spies the department store Santa Claus get drunk before taking part in the Macy's Thanksgiving parade, he immediately locates and complains to Karen Walker (Jane Alexander), the parade director. She promptly fires her Santa Claus and the old man, who turns out to be named Kris Kringle, volunteers to take his place for the sake of the children. Kris does so well during the parade that he is immediately hired to be the store's main Santa for the holiday period. After miraculously recovering from a bullet wound to the head, Gulf War veteran Jack Starks (Adrien Brody) returns to Vermont in 1992, suffering from periods of amnesia. While walking, he sees a young girl, Jackie (Laura Marano), and her alcoholic mother (Kelly Lynch) in despair beside their broken-down truck. Starks and Jackie quickly form a certain affinity; she asks him to give her his dogtags and he does so. He gets the truck started for them and continues on his way. Shortly after, a man driving along the same highway gives Jack a ride and they get pulled over by a policeman. The scene changes: Starks is found lying on the deserted roadside near the dead policeman, with a slug from the policeman's gun in his body. The murder weapon is on the ground nearby. Although he testifies there was someone else at the scene, he is not believed because of his amnesia. Starks is found not guilty by reason of insanity and is incarcerated in a mental institution.
