As the movie opens, Sherlock is in pursuit of the criminal Professor Moriarty (Vincent D'Onofrio) and apparently shoots him to death. His body cannot be found, however, as it falls in a sewer. After this incident, Holmes gains notoriety with the press and the police for his apparent killing of Moriarty, and meets Dr. Watson (Roger Morlidge), an early practitioner of autopsies, for the first time. Together, they start an investigation into the murder of several crime lords and become convinced that Moriarty is alive and behind a plot to organize drug dealing. Given that Moriarty is supposedly dead, Holmes finds it hard to convince Inspector Lestrade (Nicholas Gecks) of this claim. Holmes' investigation leads him to a prostitute (Gabrielle Anwar), who posed as a rich woman to lure Holmes into the plot. Richard E. Grant also appears as Holmes' brother, Mycroft, who was addicted to opium by Moriarty when Sherlock was young.

The film begins with the Spice Girls performing "Too Much" on Top of the Pops, but they later become dissatisfied with the burdens of it. Meanwhile, sinister newspaper owner Kevin McMaxford (Barry Humphries) is attempting to ruin the girls' reputation for his newspaper's ratings. McMaxford dispatches photographer Damien (Richard O'Brien) to take pictures and tape recordings of the girls. Less threatening but more annoying is documentarian Piers Cuthbertson-Smyth (Alan Cumming), who stalks the girls along with his camera crew, hoping to use them as subjects for his next project. At the same time, the girls' manager, Clifford (Richard E. Grant), is fending off two over-eager Hollywood writers, named Martin Barnfield and Graydon (George Wendt and Mark McKinney), who relentlessly pitch absurd plot ideas for a feature film for the Spice Girls.

The film begins with a title card describing Holmes and Watson as "ageless", as an explanation as to why the film is set in the 1940s rather than Holmes' era of 1881–1914, as the preceding 20th Century Fox films were. Though there is a nod to the classic Holmes in a scene where Holmes and Watson are leaving 221b Baker Street, and Holmes picks up his deerstalker. Watson protests, and Holmes reluctantly puts on a fedora instead.

The film begins with Moriarty and Holmes verbally sparring on the steps outside the Old Bailey where Moriarty has just been acquitted on a charge of murder due to lack of evidence. Holmes remarks, "You've a magnificent brain, Moriarty. I admire it. I admire it so much I'd like to present it, pickled in alcohol, to the London Medical Society". The film depicts the lives and misadventures of two "resting" (struggling and unemployed) young actor friends in late-1969 London. They are the flamboyant alcoholic Withnail and "I" (named "Marwood" in the published screenplay but not in the credits) as his more level-headed, anxiety-prone, and ennui-crippled friend and the film's narrator. Withnail is filled with indignation over life’s injustices, despite his privileged background. He rages against the slings and arrows of outrageous fortune all the more because he blames others for the adverse consequences of his exuberant arrogance, flagrant narcissism and habitual lying, alcoholism, and drug use. Withnail sets the tone for the friendship, with Marwood going along with whatever Withnail wants to do. They live in a filthy Georgian flat in Camden Town. While they wait for a part, daily life revolves around getting coins to use in the meters that provide gas or electricity, going to collect benefits, and waiting for the pubs to open so they can drink and be somewhere with heating. Their only other company at the flat besides each other is the local drug dealer, Danny; a somewhat distasteful man with far out, often bizarre viewpoints on the current state of affairs and a knack for irritating Withnail.

Teenagers Sherlock Holmes (Nicholas Rowe) and John Watson (Alan Cox) meet and become good friends as students at London's less-than-prestigious Brompton Academy. Watson is introduced to Holmes’ mentor, Rupert T. Waxflatter (Nigel Stock), a retired schoolmaster and inventor. Waxflatter's niece Elizabeth Hardy (Sophie Ward) is also Holmes’ close friend and love interest. Holmes is generally recognized as brilliant but considered an undisciplined troublemaker by most of the school administration. He is closest to Professor Rathe (Anthony Higgins), his fencing instructor, who warns Holmes that he is too emotional and impulsive.
