In a long corridor of an underground bunker, several prisoners are marching under armed guard. As they approach the door at the end of the corridor, one prisoner seizes his chance to assault a guard, but the door opens almost immediately and several German soldiers step through to mow down the entire group, save for one prisoner who remains standing. The guards grab him and force him through the door, where he is placed on an operating table and held down as a drip is inserted into his arm and blood pumped into his veins. He stops moving for a short time before his eyes blink open revealing his irises have become zombified-white.

In 1942 in the Swedish province of Värmland bordering Nazi-occupied Norway, a group of Swedish soldiers leave their post at a check-point in neutral Sweden, and ends up on the wrong side of the border. One soldier is killed and another captured by the Germans, and a rescue operation is underway. However, a Swedish colonel sends out an execution squad to eliminate the lost soldiers and their rescuers, in order to cover up the mistake. The lost Swedish soldiers now suddenly face two different enemies on their way home across the border.

On February 6, 1846, at Paradise Square in Lower Manhattan's Five Points, a territorial battle of hand-to-hand combat between Bill "the Butcher" Cutting's U.S.-born nativist gang, the Natives, and "Priest" Vallon's Irish Catholic immigrant gang, the Dead Rabbits, concludes when Cutting kills Vallon, witnessed by Vallon's young son, Amsterdam. Cutting declares the Dead Rabbits outlawed but orders that Vallon's body be buried with honor. Following from the previous film, it shows Eastland walking freely on the streets of New York, without any hint that his dual identity was compromised. He meets up with another old army buddy, Be Gee (Faison), who owns a garbage truck. As seen at the beginning of the film, Eastland wears a welders' mask and wields a flame thrower, while listening to a police scanner for possible crimes to stop. Slaying the brother of a gang leader named X (Van Peebles), the Exterminator gains the gang's enmity. Coincidentally, his army buddy happens to see the gang during a robbery of an armoured car, and scares them away with his truck. However, they get the truck's plate numbers, and vow revenge. Following the truck one night when the buddy loans it to Eastland, they follow Eastland to his home, and, not having seen who the driver of the truck was the night it scared them away, they presume Eastland was the man behind the wheel that night. They attack Eastland's girlfriend in the park, crippling her. Later, they break into her apartment and kill her.
