In New York City in 1975, after serving only five years of a 30-year prison sentence, Carlito Brigante is freed on a legal technicality exploited by his lawyer, Dave Kleinfeld, infuriating the district attorney. Brigante returns to his old neighborhood of Spanish Harlem, where he reconnects with old associates. Although he vows that he is finished with crime, Brigante is persuaded to accompany his cousin Guajiro to a drug deal at a bar. Guajiro is betrayed and killed while Brigante is forced to shoot his way out. He takes Guajiro's money and uses it to buy into a nightclub, with the intent of saving $75,000 to retire to the Caribbean.

Corky (Gina Gershon), an ex-con who has just finished a five-year jail sentence, arrives at an apartment building to start work as a painter and plumber. On her way up to the apartment, she encounters the couple who live next-door, Violet (Jennifer Tilly) and Caesar (Joe Pantoliano). After Caesar has gone out, Violet flirts with Corky and asks her to help retrieve an earring that has fallen down her sink. After Corky extracts the earring, Violet admits she lost it on purpose in order to get closer to Corky, and starts to seduce her. They are interrupted by the arrival of Caesar and Corky goes back to work. When she leaves for the day, Violet follows her to her truck. They go to Corky's apartment and have sex. The next morning, Violet tells Corky that Caesar is a money launderer for the Mafia and they have been together for five years.

The film begins in late 1999. The story begins in the 1960s with three inmates in a New York prison — Earl (Van Peebles), Rocco (Kelly), and Carlito (Hernandez) — controlling their criminal empire within their cell. Upon their release, they all look to control the drug trade in Harlem, which is currently in a power dispute between the Italian Bottolota crime family and black gangsters led by Hollywood Nicky (Combs). Rocco takes them to Artie Bottolota (Young) who at first is reluctant to work with blacks and Puerto Ricans, but who eventually cuts a deal with them in heroin distribution. The friends also meet Artie's son, Artie Jr. Soon, Earl's troubled younger brother Reggie joins them. After Artie Jr. offers to shake hands with Reggie, when he sees he and Carlito at a strip joint, Reggie turns him down, saying "my brother was doing fine without you". Artie Jr. insults Reggie with a racial slur, spurring Reggie to curse him and spit in his face. Artie Jr. assaults Reggie, but the fight is broken up. Carlito urges Reggie to wait until Artie Jr. and his crew leave. The mobsters leave, only for Carlito and Reggie to be cornered by them outside.
