It is based on the 1989 book Diana's Story by Deric Longden, who co-wrote the script with Jack Rosenthal. It tells the story of his marriage to his wife, Diana, who contracts a chronic, degenerative illness that medical officials were unable to understand at the time (now believed to be Myalgic Encephalomyelitis). As Diana's health faltered, she secretly set him up with another woman to help ease his pain over her eventual death.

The film opens in 1934 with Winston Churchill deep in his wilderness years, and struggling to complete his biography of his ancestor the Duke of Marlborough, which he hopes will revive his fortunes. Winston is chided by his wife Clemmie for their lack of money and is aware that as a 'man of destiny' his moment may have passed. At the same time he struggles in the House of Commons as a backbencher to get a hearing for his concerns about German re-armament under Hitler and the policy of appeasement.

Alex Carver is walking his dog and getting coffee for his wife, Ruth. Through Alex’s narrative, it is known that they are moving soon and that an open house is scheduled for their apartment tomorrow. In New York City in 1953, Ann Deveraux lives with her bandleader husband Danny, their 15-year-old son George, and Ann's somewhat older effeminate son Robbie. Ann does not seem very interested in her children's lives; she does not even remember which school they attend. Peter Colt (Paul Bettany), a British professional tennis player in his thirties whose ranking has slipped from 11th to 119th in the world, considers that he never really had to fight for anything as his wealthy, but not close, family easily put him through studies and allowed him to pursue his tennis ambitions. The film charts the history of "Flame" a fictitious group in the late 1960s who are picked up by a marketing company and taken to the top, only to break up at their zenith. For two years, Tom and Amy Bates have been struggling to cope with their altered lives, after their daughter Pattie (or Patricia) was severely injured in a hit-and-run accident. Pattie is unable to walk, completely dependent upon others for the activities of daily living, and seemingly unable to communicate beyond making unintelligible sounds. Although poorly educated and gullible, Amy Bates firmly believes that Pattie is able to understand what is being said in her presence, whereas Tom Bates has given up all hope of her recovery. In fact, judging from the sounds she makes, Pattie seems to realise what is going on around her, but Tom Bates is beyond noticing.

Emily Delahunty (Maggie Smith) is an eccentric British romance novelist who lives in Umbria in central Italy. Mrs. Delahunty settled in Italy to flee from a somewhat traumatic past which still haunts her, and lives alone apart from her servants, who put up with her eccentricities.
