The film opens with a news report on "Natas", a strange drug which has been reported to induce zombie-like effects in those who consume it. The film then fast forwards in time, to an Earth mostly inhabited by zombies. The Hunter (Martin Copping) drives along a post-apocalyptic wasteland and meets a hostile "Death Angel". The zombie is reduced to purplish goo after Hunter rams his car at it. He proceeds to stop at a deserted petrol station to fill his car's empty gas tank. Hunter enters the station's convenience store to check out any available supplies, but is ambushed by invading zombies. He eliminates them all after a gory battle and downs his sorrows by gulping a bottle of tequila. It is revealed that Hunter's daughter and wife have both died. Resuming his journey, Hunter unknowingly ventures into the turf of the survivors, who have not been infected by Natas. Thinking Hunter is a zombie, an unknown assailant takes a shot at him. Hunter survives nevertheless and is greeted by survivor Fast Lane Debbie (Jade Regier) upon regaining consciousness.

After his best friend dies in an unexpected accident, black market dealer Young-gyu decides to wash his hands off his illegal activities and start a new life. He later learns that Yu-ri needs money for her father's surgery and she has resorted to procuring the services of a local organ broker, who is Young-gyu's former client. The broker promises to help her secure a legal organ donation, but secretly relies on black market dealers like Young-gyu, who engage in multiple kidnappings and killings. To help her out, Young-gyu agrees to do the job one last time when he is once again approached by his former client. His smuggling ring gets back together for a final run and they resume their M.O. of operating out of a ferry boat that runs between Korea and China, picking victims from among its passengers, abducting them from their rooms, harvesting their organs on board the ship while they are still alive yet restrained, and then brutally disposing of the bodies afterwards.

The story of Roulette centers on three characters; Dean Jensen (Mike Baldwin), Richard Kessler (Will Haza), and Sunshine ‘Sunny’ Howard (Ali Lukowski). The three characters all suffer from suicidal thoughts brought on by depression and past events, later discovered to be connected through a network of gradually revealed ties. Meeting in a group therapy session, the three decide to ditch the group and retreat to Dean’s house where they subsequently engage in a dangerous and dramatic game of Russian Roulette. As the film progresses, the viewer is introduced to each character’s backstory, bringing to light the reasons for their depression, the actions that brought them together, and the unforeseen connections between them.
