Mia DuBois (Monica Calhoun) is a psychologist who until recently worked with the police department, counseling the survivors of victims of violent crime. While Mia has entered into a successful private practice, she's persuaded by her former colleagues to take on new client, Tammy (Chrystale Wilson), who is still dealing with the recent murder of her husband. As Mia helps Tammy with her problems, Mia finds herself thinking about her own marriage to Victor (Kristoff St. John), which has hardly been happy lately. As Mia begins wondering if she has other options, she visits a mysterious nightclub, Pandora's Box, where she meets the sexy and mysterious Hampton Hines (Michael Jai White). Mia soon begins having an affair with Hampton, unaware that Hampton is actually working with Victor and Tammy; Mia is soon to inherit $20 million, and Victor is determined to get his hands on the money. Hampton soon shifts his alliances to stay with Mia, but Victor and Tammy are not giving up their shares of the fortune quite so easily.

The film focuses on a small group of overzealous scientists who hope to use Dracula's desiccated—but still alive—body to discover the secret of immortality. Elizabeth Blaine, working at the New Orleans morgue, receives Dracula's 'corpse' from her friend and co-worker Luke following the events of Dracula 2000. (This is a departure from the epilogue of the first film, in which Mary Van Helsing explains in a voiceover that she had returned Dracula to London and assumed her father's duties as Dracula's keeper.)
The film picks up where A Distant Thunder left off. Patty Myers (played by Patty Dunning) is faced with the decision to either accept the mark of the beast or lose her head. The soldiers strap her to the guillotine, but then flee due to a sudden earthquake. Left alone, Patty first screams for help, then that she wants the mark, only to die anyway as the earthquake causes the blade to fall on its own. Nearby, a woman named Kathy (played by Susan Plumb) and David Michaels (played by William Wellman Jr.) have just escaped a government detention center during the earthquake along with Kathy's young son Billy, and another woman named Leslie (played by Wenda Shereos).
In October 2004, a New York Army National Guard unit prepares for an upcoming deployment to Iraq. First Lieutenant Danny Selfton, the executive officer of his company, was transferred to a non-deploying public affairs unit due to influence from his politically powerful father but is present to assist in the final preparations. Specialist Reyes, a combat medic in the same unit, applied for a compassionate reassignment due to his son suffering from lung cancer. However, his request for reassignment was denied by his battalion commander Lieutenant Colonel Owens due to his needed medical skills for the upcoming deployment.
Terroll Lessor, a computer genius in an unpleasant job, decides to use his skills against reprehensible parts of society. Along with his girlfriend, played by Ali Larter, he gains embarrassing information on people whom he believes to be immoral and exposes this information to the public. This garners the attention of police and people who wish him harm, and results in copy-cat crimes by others who admire him.
