Adrift begins with the wedding of Duyen and Hai. Duyen is a beautiful tourist guide and translator who marries Hai, a taxi driver who is two years her junior. Duyen thinks the marriage will make her happy but it turns out more difficult, especially since her childlike husband is uninterested in her efforts of intimacy and the marriage remains unconsummated. Cam, secretly in love with Duyen, is bitter about the marriage. Cam has Duyen deliver a letter to Tho, a handsome lothario and tourist guide who physically hurls himself on Duyen when she visits his home to deliver the letter. After Tho reports back to Cam that Duyen is sexually inexperienced, Cam diabolically persuades Duyen to take a temporary job as a translator on one of Tho’s beach tours, knowing what will happen. Duyen's time at the beach with Tho results in an affair leading to Duyen's sexual awakening but continued uncertainty about herself, marriage, and life.

The story relates the destiny of a young, beautiful and talented Vietnamese woman who sacrifices herself for her family. Kieu offers herself in payment for her father's debt without fully understanding the ramifications of the decision. Du's poem was written as an allegory for Vietnam, which has often been possessed and abused by others.

The story begins in Ha Dong, northern Vietnam in 1954 (now part of Hanoi), amidst crumbling French colonial rule in Vietnam. Dan and Gu are lovers, as well as servants in different households who suffer at the hands of their cruel masters. After Gu's master is assassinated, Dan and Gu flee south, eventually ending up in the central Vietnamese seaside town of Hoi An. There they raise a family, with Dan eventually giving birth to four girls. Although impoverished, the family love and support each other, even as the horrors of encroaching war threaten to tear them apart.

The movie is about an 18-year-old boy who has been orphaned after his father died from a truck crash while he was in his usual work. Living with the boy in a small house, there are his old grandfather, who repairs tires despite of his failing health, his little sister, who shines shoes for restaurant customers in the neighborhood, and his older sister, who carries water at a local market.

Events depicted in the movie unfold in the late 1960s and early 1970s. The Americans are bombing Vietnamese towns and villages, breaking into households, killing women and children. All ships of the European merchant fleet have already left the coast of North Vietnam, but the Soviet dry cargo ship Chelyabinsk refuses to leave Vietnamese shores despite a United States military ultimatum. A few days later the ship is sunk and surviving Soviet sailors join a caravan of Vietnamese insurgents moving along the Ho Chi Minh Trail.
