Dr. Richard Kimble, a prominent Chicago vascular surgeon, arrives home to find his wife Helen fatally wounded by a one-armed man. Kimble struggles with the killer but he escapes. The lack of evidence of a break-in, Helen's lucrative life insurance policy, and a misunderstood 9-1-1 call result in Kimble's conviction of first-degree murder. Being transported to death row by bus, his fellow prisoners attempt an escape. The pandemonium sends the bus down a ravine and into the path of an oncoming train. Kimble escapes the collision and flees. Deputy U.S. Marshal Samuel Gerard and his colleagues Renfro, Biggs, Newman and Poole arrive at the crash site and begin the search for Kimble. Kimble sneaks into a hospital to treat his wounds and alter his appearance. He eludes the authorities, but Gerard corners him at the edge of a storm drain over a dam. Kimble leaps into the raging water and escapes.

In April 2054, Washington, D.C. has a special police force called PreCrime that stops murderers before they act. Murders are predicted using three Precogs, mutated humans who "previsualize" crimes by receiving visions of the future. Over the past six years, PreCrime has successfully reduced the city's murder rate to zero, and the Federal government is on the verge of adopting it, even though the public finds it controversial. Captain John Anderton (Tom Cruise), the head of PreCrime, has been suffering from drug addiction since the unresolved disappearance of his son, Sean, which led to his wife, Lara (Kathryn Morris), divorcing him. With PreCrime poised to go nationwide, the system is being audited by Danny Witwer (Colin Farrell), an agent from the United States Department of Justice. In the middle of the audit, the Precogs generate a new prediction, one saying that Anderton will murder a man named Leo Crow in thirty-six hours. Anderton does not know Crow, but flees the area as Witwer begins a manhunt.

In an unnamed American city, soon-to-be-retiring Detective William R. First, they discover an obese man who was tied to a chair and forced to eat food, and eventually, his own vomit, until his stomach exploded (representing gluttony). Next, they investigate the fatal bloodletting of a rich attorney, from whom a pound of flesh has been extracted (representing greed). Two days later, fingerprints left at the crime scenes lead them to an apartment, where they find an emaciated man strapped to a bed. Though he initially appears to be dead, they find he has been kept alive and immobile by the killer for one year to that day; a drug dealer and child molester before his capture (representing sloth). Though unable to learn anything from the insensate man, the detectives agree the killer has planned these crimes for more than a year.

The subsequent raid on the Klan members proves to be a disaster; malfunctioning radios delay back-up, causing a firefight when Mike accidentally shoots Marcus in the buttocks, while the Klan members are revealed to be mere small-time buyers and not the distributors of the ecstasy. The incident leaves Marcus to further question if he still wants to partner with Mike, while Mike fears that Marcus may discover his new relationship with Marcus' sister, Syd.

In 2084, Earthbound construction worker Douglas Quaid is having troubling dreams about Mars and a mysterious woman there. His wife Lori dismisses the dreams and discourages him from thinking about Mars, where the governor, Vilos Cohaagen, is fighting rebels while searching for a rumored alien artifact located in the mines. At "Rekall", a company that provides memory implants of vacations, Quaid opts for a memory trip to Mars as a secret agent fantasy. However, during the procedure, before the memory is implanted, something goes wrong, and the story diverges between the question of what is real and what is hallucination. Apparently, Quaid starts revealing previously suppressed memories of actually being a secret agent. The company sedates him, wipes his memory of the visit, and sends him home. On the way home, Quaid is attacked by his friend Harry and some construction coworkers; he is forced to kill them, revealing elite fighting-skills. He is then attacked in his apartment by Lori, who reveals that she was never his wife; their marriage was just a false memory implant and Cohaagen sent her as an agent to monitor Quaid. He is then attacked and pursued by armed thugs led by Richter, Lori's real husband and Cohaagen's operative.

In the 1990s, U.S. National Security Agency official Thomas Bryan Reynolds (Jon Voight) meets with congressman Phil Hammersley (R-NY) (Jason Robards) in a public park to discuss support for new counter-terrorism legislation the U.S. Congress is pushing that dramatically expands the surveillance powers of intelligence agencies on individuals and groups. Hammersley remains committed to blocking its passage, since he believes it would almost totally destroy the privacy of American citizens. Reynolds has his team murder Hammersley, spread heart pills over his car, place him in the car and push it in a lake to make it look like he had a heart attack. In the aftermath, they discover too late that wildlife researcher Daniel Zavits (Jason Lee) had a camera aimed in the woods at their location. Zavits inspects the footage, transfers the video to an innocuous video game disc and flees the apartment ahead of Reynolds's team.

In the Mediterranean Sea, Italian fishermen rescue an unconscious American man (Matt Damon) floating adrift with two gunshot wounds in his back. When he regains consciousness, the man discovers he has dissociative (or psychogenic) amnesia. Much later he remembers fragments of the C.I.A. trauma-based programming which erased and replaced his identity. When the ship docks, he sets off to investigate the deposit box.

The story begins with an adult Frank Walker telling an off-screen audience about his attendance at the 1964 New York World's Fair as a child. In the flashback, he meets David Nix, an official who is unimpressed with Frank's limited jet pack. Frank does draw the attention of a young girl named Athena. Seeing his potential, Athena gives Frank a pin embossed with a "T" symbol and tells him to follow her aboard the fair's "It's a Small World" attraction. Frank sneaks onto the ride, where his pin is scanned and he is transported into a futuristic cityscape known as Tomorrowland. He falls from a ledge, but straps on his jet pack and lands safely before Nix and Athena.
