Bastian Balthazar Bux (Barret Oliver), a shy and friendless bibliophile child, hides in a bookstore, interrupting the grumpy bookseller, Mr. Coreander (Thomas Hill). Bastian asks about one of the books he sees, but Mr. Coreander advises against it; despite which, Bastian seizes the book, leaving a note promising to return it, and hides in the school's attic to read. The book describes the world of Fantasia threatened by a force called "The Nothing"; where the Childlike Empress (Tami Stronach) who rules over Fantasia has fallen ill, and has summoned Atreyu (Noah Hathaway) to discover the cure. Atreyu is therefore given the AURYN. As Atreyu sets out, the Nothing summons Gmork (voiced by Alan Oppenheimer), a werewolf, to kill Atreyu.

Within an uninhabited citadel (the “Keep” of the title) in World War II-era Romania lies entrapped a dangerous entity named Radu Molasar. The inner walls of the citadel contain 108 T-shaped icons, supposedly made of nickel. When the German Army under the command of Capt. Klaus Woermann occupies the castle to control the Dinu Mountain Pass following the commencement of Operation Barbarossa, Molasar is unleashed by a pair of looting soldiers who identify one glowing icon as being made of silver. In the ensuing days, Molasar kills several soldiers. A detachment of Einsatzkommandos under the command of sadistic SD Sturmbannführer Eric Kaempffer then arrives to deal with what is thought to be partisan activity, executing villagers as collective punishment.

In modern times, an antiques dealer (Henrik Galeen) finds a golem (Paul Wegener), a clay statue brought to life by a rabbi four centuries earlier. The dealer resurrects the golem as a servant, but the golem falls in love with the dealer's wife. As she does not return his love, the golem commits a series of murders.

Set in Jewish ghetto of medieval Prague, the film begins with Rabbi Loew, the head of the city's Jewish community, reading the stars. Loew predicts disaster for his people and brings his assistant to inform the elders of the community. The next day the Holy Roman Emperor signs a royal decree declaring that the Jews must leave the city before the new moon. The Emperor sends the knight Florian to deliver the decree. Loew meanwhile begins to devise a way of defending the Jews.

In a Prague ghetto, poor Jews find themselves oppressed by Emperor Rudolph II (Harry Baur) which leads to talk among the Jews of re-awakening the Golem who is being held in an attic by Rabbi Jacob (Charles Dorat). During a food riot Rudolph's mistress, the Countess Strada (Germaine Aussey), is rescued by the enamored De Trignac (Roger Cuchesne) who gets hurt in the process. De Trignac is taken to Rabbi Jacob's house by his wife Rachel (Jany Holt). When Rudolph gets engaged to his cousin Isabel of Spain, it angers Strada who charms De Trignac to steal Jacob's Golem.
