In Rome, American diplomat Robert Thorn (Gregory Peck) is in a hospital where his wife Katherine (Lee Remick) gives birth to a boy, whom he is told dies moments after being born. Robert is convinced by the hospital chaplain, Father Spiletto (Martin Benson), to secretly adopt an orphan whose mother died at the same time. Robert agrees, but does not reveal to his wife that the child is not theirs. They name the child Damien (Harvey Spencer Stephens).

The film opens with a short fragment outside the plot but clearly related on repeated viewings. Grainy, black-and-white, and silent, a title "Once Upon a Time" leads to Latino labourers picking coffee beans while armed foremen push rudely between them. One worker (McDowell with black hair and moustache) pockets a few beans ("Coffee for the Breakfast Table") but is seen by a foreman. He is next seen before a fat Caucasian magistrate who loses some saliva as he removes his cigar only to say "Guilty." The foreman pulls his machete and lays it across the unfortunate laborer's wrists, bound to a wooden block, revealing that he is to lose his hands for the theft of a few beans. The machete lifts, descends, and we see McDowell draw back in a silent scream. The scene blacks out, the word NOW appears onscreen and expands quickly to fill it.

Whilst driving his Rover P5B, uptight City worker Harold Pelham appears to become possessed and has a serious high-speed accident. On the operating table, he briefly suffers clinical death, after which there appear to be two heartbeats on the monitor. When he awakes, Pelham finds his life has been turned upside-down; in his job as a director of a marine technology company he learns that he now supports a merger that he once opposed, and that he apparently is having an affair. Set in a British public school (a fee-paying boarding school) in the late 1960s, the film opens as the pupils return for a new term. Mick Travis (Malcolm McDowell), Wallace (Richard Warwick), and Johnny (David Wood) are three non-conformist boys in the lower sixth form (the penultimate year). They are watched and persecuted by the "Whips", upper sixth formers given authority as prefects over the other boys, while younger boys (whom the prefects degradingly call "Scum") are made to "fag" (act as personal servants) for the Whips.

The seven Hook children, whose ages range from five to fourteen, live in a dilapidated Victorian house in suburban London. The older children help to care for their invalid single mother, whose chronic illness has led to her to convert to fundamentalist religion and refuse all medical help. When their mother dies suddenly, the children realise that they may be split up and sent to orphanages, so they decide to conceal their mother's death and carry on with their daily routine as if she were still alive. Cardinal Wolsey, the Lord Chancellor of England, summons Sir Thomas More to Hampton Court. King Henry VIII of England wishes to divorce his wife and marry Anne Boleyn. Saying that England needs a male heir to prevent another civil war, Wolsey chastises More for being the only member of the Privy Council to argue against him.
