Following the events of Harold & Kumar Go to White Castle, Harold Lee and Kumar Patel set off on a flight to Amsterdam so that Harold can pursue a budding romance with his neighbor, Maria. Before they board their plane, they run into a friend from college named Colton, and Kumar's ex-girlfriend, Vanessa, who are getting married in Texas. During the flight, a woman suspects Kumar is a terrorist after mistaking his bong for a bomb. Sky marshals detain the duo and the plane diverts to Washington D.C. Ron Fox, an obsessive and racist Deputy Secretary of Homeland Security, sends them to Guantanamo Bay detention camp in Cuba, thinking they are agents of a joint Al-Qaeda and North Korean conspiracy, against the advice of NSA Vice-Chairman John Beecher. With the inadvertent assistance of two other prisoners, Harold and Kumar escape and board a Cuban refugee boat to Miami, Florida. They meet up with their college friend, Raza, who loans them a car so they can travel to Texas to get help from Colton, whose family has political connections.

In rural Texas, go-go dancer Cherry Darling (Rose McGowan) decides to quit her low-paying job and find another occupation. She runs into her mysterious ex-boyfriend El Wray (Freddy Rodriguez) at the Bone Shack, a BBQ restaurant owned by J.T. Hague (Jeff Fahey) and his sheriff brother (Michael Biehn). Meanwhile, a group of officials at a nearby US military base, led by the demented Lt. Muldoon (Bruce Willis), are making a business transaction with a chemical engineer named Abby (Naveen Andrews) for mass quantities of a deadly biochemical agent known as DC2 (codename "Project Terror"). When Muldoon learns that Abby has an extra supply on hand, he attempts to take Abby hostage, and Abby intentionally releases the gas into the air. The gas reaches the town and turns most of its residents into deformed bloodthirsty psychopaths, referred to as "sickos" by the surviving humans. A U.S. Army librarian, Corporal Joe Bauers (Luke Wilson), and a prostitute, Rita (Maya Rudolph), are selected for a suspended animation experiment on grounds of 'average' appearance, intelligence, behavior, etc. Rita's pimp "Upgrayedd" (Brad Jordan) has been bribed to allow her to take part. Joanna Mills (Sarah Michelle Gellar), a travelling rep for a trucking company, is dedicated to her successful career but something of a loner. Since the age of 11 she has been a troubled person, with episodes of self-mutilation and menacing visions. Normally she avoids returning to her native Texas, but agrees to a trip there to secure an important client. During the trip her visions, which take the form of memories of events not from her life, increase in intensity. She sees a strange face staring back at her in the mirror. Her truck radio plays Patsy Cline's "Sweet Dreams" no matter what station she selects. She stops at the scene of an accident that, on the following day, seems not to have happened. Joanna cuts herself in a bar restroom and is narrowly rescued by a friend. She visits her father, who observes that from age 11 she was "a different girl". The visions continue, becoming both more specific and more threatening, centering upon a menacing man she does not recognize and a bar she has never seen, but a picture of which is in one of her catalogs.

Steve Barker (Johnny Knoxville) hates his job but after over two years of working, receives a promotion. His first duty is to fire his friend Stavi, who is the janitor. Steve fires Stavi (Luis Ávalos), but hires him to work around his apartment. Stavi gets three fingers cut off in a lawn-mower accident, and reveals that he doesn't have health insurance. Steve must raise $28,000 within two weeks to pay for the surgery to re-attach his friend's fingers. His uncle Gary (Brian Cox), owes $40,000 in gambling debts and suggests that they fix the Special Olympics in San Marcos, Texas in order to solve both of their financial problems. Steve, who competed in track and field in high school as well as being in the drama club, enters the Special Olympics in the guise of a high functioning young man with learning difficulties named Jeffy Dahmor. Gary, assuming that Steve will easily defeat the legitimate contenders, bets $100,000 that reigning champion Jimmy Washington won't win the gold medal. Despite initially being disgusted at pretending to be mentally challenged, Steve goes along with it for Stavi.

At the beginning of the film, two lonesome Texas Rangers, Roland Sharp (Tommy Lee Jones) and Maggie Swanson (Liz Vassey), are going to a church in order to question Percy Stevens (Cedric the Entertainer) about the whereabouts of his former prison roommate, Morgan Ball, who they want to testify against organized crime boss John Cortland. Percy is indignant, telling Sharp and Swanson that he is a "man of God" and hasn't spoken with Ball in years. However, Percy's cellphone rings, displaying Ball's name. Sharp and Swanson track down Ball to the warehouse, where Ball gives Sharp a key in an attempt to buy him off. Instead, Sharp takes the key and forces Ball outside, where FBI agent Eddie Zane (Brian Van Holt) is waiting. As they talk, a sniper begins shooting, wounding Swanson and giving Ball a chance to escape. It is revealed the sniper is after Ball, and a group of cheerleaders from the University of Texas at Austin witness his murder. Agent Zane is found shot in the arm next to Ball's body and claims he didn't see the sniper.
