L'histoire se déroule quatre ans après le film. Allen Bauer et sa femme Madison (une sirène) vivent sur île déserte méconnue. Allen s'ennuie : Freddie, son frère et la vie new-yorkaise lui manquent. Madison a un pouvoir hydrokinétique qui lui permet de voir et de communiquer à distance en traçant un cercle à la surface de l'eau. Consciente de la tristesse d'Allen, elle lui montre ce qui se passe à New-York. Allen réalise alors que l'entreprise familiale qu'il a laissée à son frère est en fâcheuse posture. Il décide alors de retourner à la civilisation (ce qui contredit la fin du film de 1984, où l'on apprenait que si Allen choisissait la mer, il ne pourrait jamais revenir).

Arthur Bach (Dudley Moore) is a spoiled alcoholic from New York City who likes to be driven in his chauffeured Rolls-Royce through Central Park. Arthur is heir to a portion of his family's vast fortune, which he is told will be his only if he marries the upper class Susan Johnson (Jill Eikenberry), the daughter of a business acquaintance of his father. He does not love Susan, but his family feels she will make him finally grow up. Max Bialystock (Zero Mostel) is a washed-up, aging, and also fraud and corruptible greedy Broadway producer who ekes out a living romancing lascivious wealthy elderly women in exchange for money for his next play. Accountant Leopold "Leo" Bloom (Gene Wilder) arrives at Max's office to do his books and discovers there is a $2,000 discrepancy in the accounts of Max's last play. Max persuades Leo to hide the relatively minor fraud, and, while shuffling numbers, Leo has a revelation: a producer could make a lot more money with a flop than a hit, by overselling the shares in the production as no one will audit the books of a play presumed to have lost money. Max immediately puts this scheme into action. They will over-sell shares on a massive scale, and produce a play that will close on opening night, thus avoiding a pay-out and leaving the duo free to flee to Rio de Janeiro with the profits. Leo is afraid such a criminal venture will fail and they will go to prison, but Max eventually convinces him that his drab existence is no better than prison.

The film tells the story of Angie Rossini (Natalie Wood), a salesclerk at Macy's department store who finds herself pregnant after a one-night stand with musician Rocky Papasano (Steve McQueen). David Coleman (Gazzara) is a young doctor hired by a hospital's pathology department. The head of the department, Dr. Joseph Pearson (March), sees Coleman as a rival, and they fight over many medical issues. Coleman falls in love with Cathy Hunt (Balin), a nurse at the hospital, who develops a tumor in her knee. Pearson believes that the tumor is malignant and that the leg should be amputated, but Coleman disagrees. Coleman orders three blood tests on Mrs. Alexander (Love), an expectant mother whose baby may have hemolytic disease, but Pearson believes that the tests are excessive and cancels the third test. Mrs. Alexander is married to a young intern at the hospital (Clark), who, along with Coleman, tried to push for the third test. When the baby is born seriously ill, Dr. Charles Dornberger (Albert), Mrs. Alexander's OB/GYN, berates Pearson and conducts a blood transfusion to save the baby's life. Pearson's future at the hospital becomes uncertain, and he resigns. Coleman has changed his mind about Cathy's tumor and agrees with Pearson's decision, while Pearson says that Coleman reminds him of himself when he was young and urges him not to let hospital bureaucracy to wear him down.
