The film opens with two small-time crooks Tommy Uva (Pitt) and Rosie DeToma (Arianda) getting high and robbing a florist on Valentine's Day. Tommy holds up the cashier with a pistol while Rosie waited in the car she inherited from her father. Tommy is arrested and sent to prison for 18 months. In the meantime, Rosie gets a job at a debt collection agency run by Dave Lovell (Dunne), who went to jail for bilking major companies out of $800,000 through fraudulent invoices. Lovell reformed his life and mainly hires ex-cons at his firm. He is happy to give Tommy a job and a second chance. Dalton Russell (Clive Owen), seated in what appears to be a jail cell, opens the film with a prologue about having carried out the "perfect robbery". A van is driving from Brooklyn to the Wall Street area. Inside is a team of masked robbers, dressed as painters, who call each other by variants of the name "Steve" (i.e. Steve, Stevie, Stevo). They seize control of a Manhattan bank and take the employees and patrons hostage. They divide the hostages into groups and hold them in different rooms, forcing them to strip and don painters' clothes identical to their own. The robbers rotate the hostages between various rooms and occasionally insert themselves covertly into the groups, while also taking turns working on an unspecified project involving demolishing the floor in one of the bank's storage rooms.

While in post-production on a low-budget exploitation film, Philadelphia sound technician Jack Terry (Travolta) is told by his producer that he needs a more realistic sounding scream and better wind effects. After leaving the studio to record potential sound effects at a local park, he sees a car careen off the road and plunge into a nearby creek. Jack dives into the water to help, discovering a dead man and a young woman, still alive, trapped inside the submerged car. He pulls her to safety and accompanies her to a local hospital. Jack learns that the driver of the car was the governor (and a presidential hopeful); the girl was an escort named Sally (Allen). Associates of the governor attempt to whitewash the incident by concealing that Sally was in the car, and they convince Jack to smuggle Sally out of the hospital with him.

Police officers face many challenges in the decayed South Bronx region of New York City. Among them are NYPD officers Murphy (Newman) and Corelli (Wahl), who work out of the 41st precinct, nicknamed "Fort Apache" because to those who work there, it feels like an army outpost in foreign territory (an allusion to Fort Apache out of the Old West).
