Fresh out of prison, Isaiah Bone moves to Los Angeles, where underground fights are being held. One night, after watching a match involving local champion Hammerman, Bone makes a deal with promoter Pinball to get him into the fight scene for 20% of his earnings; 40% if Pinball puts his own money on the line. On that same night, Bone encounters mob boss James and his girlfriend Angela Soto. Bone enters his first underground fight and quickly defeats his opponent with only two kicks. Pinball explains to Bone that Angela was previously married, but James set up her husband Danny on a triple-homicide, sending him to jail. When James learned that she was pregnant, he had her undergo an abortion. Since then, Angela has fallen into drug addiction.

Timid accountant Jonathan McQuarry (Ewan McGregor) is an auditor working out of New York. When hip-hop star Christopher “C-Note” Hawkins (Big Boi) is denied membership into an exclusive Carolina Pines Country Club, he comes up with a cunning plan that will oblige the country club to allow his acceptance. Thomas Archer (Ron Eldard), a disconsolate man, is led to an underground room by a distorted male voice on his mobile phone. In flashback it is shown that his wife had been attacked and his son killed for no apparent reason. When he reaches the underground room he finds a man hooded and tied to a chair and told by the distorted voice that this is the man responsible for the attacks. Without any proof he accepts everything he is told and proceeds to torture the man with various implements found in the room. After a while Archer realizes that that this may not be the man responsible for the attacks. Against the orders of the voice he questions the man. They then collaborate in an attempt to escape what they now see as a situation in which the lives of both of them are at risk. It is revealed in flashback that his Doctor (Christopher Plummer) had made Archer an offer to get the revenge he needs. The doctor now reappears and tries to insist that Archer "finish it, and take back your life".

During the film's opening credits, two bookies are separately ambushed and murdered by their unseen killers, and, elsewhere, a young man is killed by a sniper. In a bus terminal, a young man is approached by Goodkat (Bruce Willis), who tells the story of Max and the Kansas City Shuffle: two decades earlier, Max borrowed money from the mob to bet on a fixed horse race, only for the horse to die mid-race. To set an example to make sure nobody else would try to bet on a fixed race, the mob killed Max, his wife and young son Henry. Goodkat concludes that a "Kansas City Shuffle" is a misleading double bluff, and so tricks and kills the young man, before loading his body into a truck.

When Bobby Saint (James Carpinello) and Mickey Duka (Eddie Jemison) meet with European arms dealer Otto Krieg at the Tampa, Florida seaport, the FBI intervenes and Saint is killed while Duka is jailed.
