Twenty years after his disappearance in 1908, an aged Butch Cassidy (Sam Shepard), living under the assumed name James Blackthorn in a secluded village in Bolivia, decides to end his long exile and return to the United States after learning of the death of Etta Place. Dr. Owen (Jonathan Mellor), an official from the Ministry of Health, and a GEO team equipped with video cameras are sent into a quarantined apartment building to control the situation. After Martos, a GEO officer, is killed and infected encountering some of the infected, Owen uses religious mantra and rosary to hold him off in a room. Ángela Vidal (Manuela Velasco), and her cameraman, Pablo, are covering the night shift in one of Barcelona's local fire stations for the documentary television series While You're Sleeping. While they are recording, the firehouse receives a call about an old woman, Mrs. Izquierdo, who is trapped in her apartment and screaming. Ángela and Pablo accompany two of the firefighters, Álex and Manu, to the apartment building, where two police officers are waiting. As they approach the old woman, she becomes aggressive and attacks one of the officers, biting his neck.

Hiver 1985. The story takes place in 1851 and deals with a small Spanish village apparently plagued by what we would now call a serial killer, as corpses are discovered bearing both savage mutilation and precise surgical incisions. Clues point toward Manuel Romasanta (Julian Sands), who confesses to the crimes, but claims that he is a victim of lycanthropy. A scientist, Professor Philips (David Gant), argues that Romasanta suffers not from a supernatural curse but from a mental disorder.
