In Budapest to intercept a courier working for an individual code-named "Cobalt", IMF agent Trevor Hanaway is killed by assassin Sabine Moreau. Hanaway's team leader, Jane Carter, and newly promoted field agent Benji Dunn extract Ethan Hunt and his source, Bogdan, from a Moscow prison. Ethan is recruited to lead Jane and Benji to infiltrate secret Moscow Kremlin archives and locate files identifying Cobalt. During the mission, someone broadcasts across the IMF frequency, alerting the Russians to Ethan's team. The modern-day descendant of Little Red Riding Hood, Virginia, brings her fiance, Nathan, home to meet her family. Virginia's brother Marcus and Jake are quick to welcome Virginia back, affectionately calling her "Red". Jake explains it is a family nickname passed down to the first daughter every generation. After the arrival of both people, Nathan finds a man staggering up the driveway half dead. The dying man says the name "Gabriel" before Nathan runs for Virginia. When they return, the man is already ashes. The sheriff arrives and contains the situation, much to Nathan's dismay. Virginia takes Nathan inside and explains that they hunt werewolves. Nathan, not believing her, goes for a walk at sunset. He is attacked and bitten by a werewolf who reveals his name to be Gabriel. The next day, while preparing for a hunt, Nathan asks if they had ever turned a werewolf back. Virginia tells him that the only way to break the curse is to kill the wolf who turned the person, before they kill a human on their first full moon. Shortly after, they go hunting in town where Nathan succeeds in killing a werewolf. After the wolves are dead, they find a girl locked in their trunk. She tells them that the wolves are planning on a "Game" that no human comes out of. Later that night, while setting up camp, Nathan transforms into a wolf for the first time. Desperate to protect him, Virginia insists on locking him up for the night. Nathan awakes the next morning in a cell. With the curse over for the night, he is released for the day, to help hunt Gabriel. Back in town, Marcus and Jake are taken captive for the new "Game". When the night comes to an end, it is revealed that the brothers were killed.

Getting ready for dinner one night, Jamie Ashen and his wife Lisa receive a mysterious ventriloquist doll named "Billy", in an unmarked package. Wondering who sent the doll, Lisa recalls a poem about a woman named Mary Shaw. Jamie disregards this and leaves to get dinner. After Lisa plays around with Billy, all the sounds disappear and something attacks Lisa. When Jamie arrives with their dinner, he hears Lisa call him into the bedroom, but becomes confused when he notices blood all over the floor. Jamie finds Lisa in their bed, dead with her tongue ripped out, and Billy lying near her body. Detective Jim Lipton brings Jamie in for questioning of the murder of his wife, but releases him due to lack of evidence. While ripping the lining of the box Billy came in, Jamie discovers that Billy belonged to Mary Shaw, a ventriloquist from his hometown of Ravens Fair.

During the film's opening credits, two bookies are separately ambushed and murdered by their unseen killers, and, elsewhere, a young man is killed by a sniper. In a bus terminal, a young man is approached by Goodkat (Bruce Willis), who tells the story of Max and the Kansas City Shuffle: two decades earlier, Max borrowed money from the mob to bet on a fixed horse race, only for the horse to die mid-race. To set an example to make sure nobody else would try to bet on a fixed race, the mob killed Max, his wife and young son Henry. Goodkat concludes that a "Kansas City Shuffle" is a misleading double bluff, and so tricks and kills the young man, before loading his body into a truck.

In 1961, the Soviet Union launches its first ballistic missile nuclear submarine, the K-19. The ship is led by Captain Alexei Vostrikov, aided by executive officer Mikhail Polenin. Polenin, the original captain, and the crew have served together for some time but Vostrikov's appointment is alleged to have been aided by his wife's political connections. During his first inspection, Vostrikov discovers the submarine's reactor officer to be drunk and asleep on duty. Vostrikov sacks the officer and orders Polenin to request a replacement. The new reactor officer, Vadim Radtchenko, arrives direct from nuclear school having just been fresh from the naval academy, annoying Polenin who thinks Vostrikov was too punitive on the former reactor officer who was competent despite his momentary lapse of judgment. Also, during the preparation period for the sub's launch, the ship's medical officer is killed when struck by an oncoming truck, and is subsequently replaced by the command's foremost medical officer, an army officer who has graciously offered himself in the submarine's time of need, but also privately admits to Vostrikov that as an army officer he has never been out to sea and suffers from motion sickness. During the K-19's official launch, the bottle of champagne fails to break when it strikes the bow; the sailors nervously glance at each other due to this customary sign of bad luck.
