The film takes place in Koishikawa, a district of Edo (the former name of the city of Tokyo), in the 19th century. Young Dr. Noboru Yasumoto (Yūzō Kayama) is the film's protagonist. Trained in a Dutch medical school in Nagasaki, the arrogant Yasumoto aspires to the status of personal physician of the Shogunate, a position currently held by a close relative; his father is already a well-established, highly competent physician. Yasumoto believes that he should progress through the safe, and well-protected, army structure of medical education. However, for Yasumoto's post-graduate medical training, he has been assigned to a rural clinic under the guidance of Akahige ("Red Beard"), Dr. Kyojō Niide (played by Toshiro Mifune). Dr. Niide may seem like a tyrannical task master, but in reality he is a compassionate clinic director. Initially, Yasumoto is livid at his posting, believing that he has little to gain from working under Akahige. Dr. Yasumoto feels that Dr. Niide is only interested in his medical notes and soon rebels against the clinic director. He refuses to wear his uniform, disdains the food and spartan environment, and enters the forbidden garden where he meets "The Mantis" (Kyōko Kagawa), a mysterious patient that only Dr. Niide can treat.

Three men, Sansai Dobe (Ganjirō Nakamura), Kawaguchiya (Saburō Date) and Hiromiya (Eijirō Yanagi) are responsible for the deaths of seven-year-old Yukitarō’s mother and father. Yukitarō is adopted and brought up by Kikunojō Nakamura (Chūsha Ichikawa), the actor-manager of an Osaka kabuki troupe. The adult Yukitarō (Kazuo Hasegawa) becomes an onnagata, a male actor who plays female roles. He takes the stage name Yukinojō. Like many of the great onnagata, particularly of the seventeenth and eighteenth centuries, he wears women’s clothes and uses the language and mannerisms of a woman offstage as well as on. Many years later, the troupe pays a visit to Edo, where the three men responsible for his parents’ deaths now live. Yukinojō brings about their deaths by means of various stratagems, then, apparently overcome by what he has done, retires from the stage and disappears, no-one knows where. The events of the film are coolly observed and sardonically commented on by the Robin-Hood-like thief Yamitarō, also played by Hasegawa.

The blind masseur Zatoichi is hired by yakuza gang-leader Sukegoro (Eijiro Yanagi) as he thinks that war is inevitable with the rival yakuza Shigezo (Ryuzo Shimada). Zatoichi has a distinguished reputation as a swordsman and Sukegoro thinks he is money well spent. Shigezo responds by hiring a ronin of similar repute, Miki Hirate (Shigeru Amachi). Ichi is a meek and humble man who is commonly underestimated by most men, and looked upon suspiciously. His being a masseur, which was a position of low regard in feudal Japan, merely increases the hostility that is shown towards him. They even try to take advantage of his blindness in a gambling den, but from early on it is clear that Zatoichi draws strength from his disability.

One year after the first film, Zatōichi travels back to the town near the Joshoji Temple, to pay respects at the grave of Hirate, the samurai he killed. Three brigands attack Zatōichi while he dries his clothes, and are despatched by a one-armed swordsman. Later that day, Zatōichi is hired to massage a powerful lord who, unbeknownst to all but the lord's highest retainers, is insane. Zatōichi observes the nobleman's unstable mental condition, and the retainers decide to kill him. Zatōichi defeats the first three attackers, and retires to a restaurant. The attack having failed, the lord's men hire local yakuza (gangsters) to finish the job. Learning of this, Zatoichi remarks to himself that he would have kept quiet if they had just asked him to do so.
