In medieval Germany, the Teutonic Knights massacre a village of supposed "witches" and build the titular structure over their dead bodies. In the present day, the church's new librarian (Tomas Arana) breaks the seal of the crypt out of curiosity and releases the evil spirits contained beneath it. At the same time, the church's automated mechanisms, which were set up by the architect who built the church, are triggered, causing the doors to close and trapping everyone inside, including a group of visitors. Father Gus (Hugh Quarshie), the only person not affected by the demons, eventually finds the way to make the church collapse on itself but dies in the process of doing so. Before the movie ends, Lotte (Asia Argento), the sacristan's daughter and the sole survivor of the incident, is seen walking towards the ruins of the church. She finds the seal of the crypt, opens it and peers inside. Blue light emits from within, just like when the librarian first opened it, and she smiles.

As an old man nearing the end of his life, Adso, youngest son of the Baron of Melk recounts how, as a young novice in 1327, he accompanied his mentor, the Franciscan friar William of Baskerville to a Benedictine abbey in Northern Italy. The abbey had been chosen as the site for a theological debate between the Franciscan order and the Pope on the poverty of Christ. The abbey is already home to a famed scriptorium where scribes copy, translate or illuminate books. The mysterious recent death of the monk Adelmo of Otranto - a young but famous illuminator - has stirred fears among the abbey's devout inhabitants. The Abbot seeks help from William, known for his deductive powers. The illuminator's death cannot be dismissed as a suicide because the body was found at the foot of a tower having only a window which cannot be opened. William is reluctant to involve himself, though he is persuaded not only because of the intellectual challenge, but also because of his desire to disprove fears of a demonic culprit in Adelmo's death. William is also motivated to act by his fears that the Abbot will summon officials of the inquisition if the mystery remains unsolved.

While Jesus is preaching with his Apostles, the confessor John the Baptist is arrested by the king of Judea to the many defamatory sermons against the power of the monarchy. Herod, son of Herod the Great, imprisons John, and Princess Salome, daughter of Herod, without him being aware of it, secretly falls in love with John the Baptist. But it is a corrupt and lustful love, which comes from lying insults that John turns to the corrupt family of Herod. When Herod, in the birthday of his daughter, asks to Salome what gift she wants, Salome says she wants to see the severed head of John. Herod the content, and so Salome, when John is beheaded, performs the dance of the seven veils, and falls into sexual rapture, kissing full of the passion the mouth of the head. Herod, horrified, puts to death his daughter.

Rose Elliot (Miracle), a poet living alone in New York City, finds an ancient book called The Three Mothers. It tells of three evil sisters who rule the world with sorrow, tears, and darkness. The book, written by an architect named Varelli, reveals that the three dwell inside separate homes that had been built for them by the architect in Rome, Freiburg, and New York. Rose suspects that she is living in one of the buildings and writes to her brother Mark (McCloskey) in Rome, urging him to visit her. Using clues provided in the book as a guide, Rose searches the cellar of her building and discovers a hole in the floor which leads to a water-filled ballroom. She accidentally drops her keys and enters the water to find them. After she reclaims the keys, a putrid corpse suddenly rises from the depths, frightening her.
