Two days after the events of the first film, a traumatized and blood-covered Sarah (Shauna Macdonald) escapes the cave with no memory of the events that occurred within it. She is taken to a hospital, where it is revealed some of the blood on her is the same type as Juno Kaplan (Natalie Mendoza). Sheriff Vaines (Gavan O'Herlihy) demands that along with his deputy Elen Rios (Krysten Cummings), Sarah and three spelunking specialists – Dan (Douglas Hodge), Greg (Josh Dallas) and Cath (Anna Skellern) – must go back into the cave to find the missing women. A new entrance is found with the help of a sniffer dog, and the team members are sent down via an old mine shaft operated by the old, mysterious Ed Oswald (Michael J. Reynolds).

Adam, an English backpacker, breaks up with his girlfriend immediately after arriving with her in Thailand. He then strikes out on his own, leaving Bangkok for Ko Samui. There, he meets Em, a young masseuse. At first, their relationship is innocent, but Adam soon grows frustrated and starts hitting the bars and becoming a sex tourist, or "butterfly man", flirting from woman to woman. The story is based on the myth of King Arthur. A young, inexperienced squire Valiant, masquerading as Sir Gawain, is sent to accompany the Welsh princess Lady Ilene, a guest at Camelot, on her way back home. Little he knows that, meanwhile, the evil sorceress Morgan le Fay has convinced the Viking warlord Sligon, ruler of the kingdom of Thule, to steal the magical sword Excalibur during a joust tournament. Valiant and the princess become part of the struggle of "he who holds the sword rules the world" which leads them both to love and Valiant to his princely destiny, as it turns out he is the rightful heir to the throne of Thule. The usurper is eventually killed by his brother Thagnar, who also captures Ilene. In a final confrontation, the villains are killed and Valiant rescues the princess and recovers Excalibur.

A prophecy states that a female child with a special birthmark will herald the downfall of the evil sorceress Queen Bavmorda. Bavmorda imprisons all pregnant women in her realm to prevent fulfilment of the prophecy. When the prophesied child is born, the mother begs the midwife to take her to safety. The midwife reluctantly accepts, and leaves Nockmaar castle unnoticed. The mother is executed, and the midwife is eventually found. Knowing she cannot escape, she sets the baby on a makeshift raft of grass and sends her down the river, hoping for fate to run its course. Bavmorda sends her daughter Sorsha and General Kael to find the baby so that the prophecy can never be fulfilled.

Paul Kersey (Charles Bronson) has come back to Brooklyn after being banned since the events of the first film to visit his Korean War buddy Charley, who is attacked by a gang in his apartment. The neighbors hear commotion and call the police. Paul arrives as Charley collapses dead in his arms. The police mistakenly arrest Paul for the murder. At the police station, Inspector Richard Shriker (Ed Lauter) recognizes Paul as "Mr. Vigilante". Shriker lays down the law before Paul is taken to a holding cell. In the same cell is Manny Fraker (Gavan O'Herlihy), leader of the gang who attacked and killed Charley. He and Paul fight. When he is released, Manny threatens Paul. Manny arrives on his “gang turf” and slashes fellow gang member Hector (David Crean), possibly for betrayal. The police receive daily reports about the increased rate of crime. Shriker offers a deal to Paul—to kill all the punks he wants, as long as he informs Shriker of any gang activity he hears about so the police can get a bust and make news.

After MI6 agent James Bond, 007, fails a routine training exercise, his superior, M, orders Bond to a health clinic outside London to get back into shape. While there, Bond witnesses a mysterious nurse named Fatima Blush giving a sadomasochistic beating to a patient in a nearby room.
