The film is divided in over 30 chapters. In the fictional Irish town of Tyrellin, near the border of Northern Ireland in the late 1940s, cartoon robins narrate as Patrick Braden's mother, Eily Bergin, abandons him on the doorstep of the local parochial house where his father, Father Liam, lives. He is then placed with an unloving foster mother. Biologically male, a young Patrick is later shown donning a dress and lipstick, which angers his foster family. Patrick is accepted by his close friends Charlie, Irwin, and Lawrence, as well as by Lawrence's father, who tells Patrick Eily looked like blonde American movie star Mitzi Gaynor.

The film revolves around the intense relationship of the two teenage protagonists, Darren (Cillian Murphy) and Sinéad (Elaine Cassidy), nicknamed "Pig" and "Runt". Pig and Runt were born at the same hospital at nearly the same time and grow up right next door to each other. This brings about a very close relationship between the two that borders on telepathic. In 1982, Johnny and Sarah Sullivan and their daughters Christy and Ariel enter the United States on a tourist visa via Canada, where Johnny was working as an actor. The family settles in New York City, in a rundown Hell's Kitchen tenement occupied by drug addicts, transvestites, and a reclusive Nigerian artist/photographer named Mateo Kuamey. Hanging over the family is the death of their five-year-old son Frankie, who died from a brain tumor. The devout Roman Catholic Johnny questions God and has lost any ability to feel true emotions, which has affected his relationship with his family. Christy believes she has been granted three wishes by her dead brother, which she only uses at times of near-dire consequences for the family as they try to survive in New York.

Gerry Conlon (Daniel Day-Lewis) is shown in Belfast stripping lead from roofs of houses when security forces home in on the district with armoured cars, and a riot breaks out. Gerry's father, Giuseppe Conlon (Pete Postlethwaite), later saves him from IRA punishment, and he is sent off to London to stay with his aunt, for his own good. Instead, he finds a squat, to explore, as he puts it, "free love and dope". One evening by chance he gains entry to a prostitute's flat and he steals the £700 he finds stashed inside; on that evening in Guildford there is an explosion at a pub that kills four off-duty soldiers and a civilian, and wounds sixty-five others.
