In a flashback, Deborah Myers (Sheri Moon Zombie) visits her son, a young Michael Myers (Chase Wright Vanek), at Smith's Grove Sanitarium. She gives him a white horse statuette as a gift. Michael says that the horse reminds him of a dream he had of Deborah's ghost, dressed in all white and leading a horse down the sanitarium halls toward Michael, telling him she was going to bring him home. Fifteen years later, after having shot an adult Michael (Tyler Mane), Laurie Strode (Scout Taylor-Compton) is found wandering around in a state of shock by Sheriff Brackett (Brad Dourif), who takes Laurie to the emergency room. Meanwhile, the paramedics pick up the Sheriff's daughter and Laurie's friend Annie (Danielle Harris) and Michael's psychiatrist Dr. Loomis (Malcolm McDowell), who are still alive after having been attacked by Michael, and take them to the hospital. Presumed dead, Michael's body is loaded into a separate ambulance. When the driver has a traffic accident, Michael awakens and escapes the ambulance, walking toward a vision of his mother dressed in white and leading a white horse.

The "Minutemen", a team of costumed crime fighters, was formed in 1939 in response to a rise in costumed gangs and criminals; the "Watchmen" was similarly formed decades later. Their existence has dramatically affected world events: in particular, Doctor Manhattan's superpowers help the United States win the Vietnam War, resulting in President Richard Nixon being reelected. Doctor Manhattan gives the West a strategic advantage over the Soviet Union, which by the 1980s threatens to escalate the Cold War into a nuclear war. During that time, growing anti-vigilante sentiment in the country leads to masked crime-fighters being outlawed. While many of the heroes retire, Doctor Manhattan and the Comedian operate as government-sanctioned agents, and Rorschach continues to operate outside the law.

In this latest interpretation, the characters Barb and her brother Johnny arrive late for their aunt's funeral and find the cemetery overrun with zombies. After Johnny abandons her, Barb flees the cemetery and is rescued by Ben, a local college student. The two seek refuge in the nearby farmhouse of the Cooper family (Henry & Hellie Cooper, Henry's daughter and Hellie's stepdaughter Karen, farmhand Owen, and farmhand Tom and his girlfriend Judy), and attempt to live through the night along with other survivors, including the pyrophobic mortician, Gerald Tovar, Jr. As Barb and Ben attempt to convince the Cooper family that the zombies are heading to the house, Tom and Judy are attacked while having sex in the barn. After hearing Judy's screams, Barb and the rest of the household attempt to save her, but they are too late. When Tovar arrives, he explains what is happening. Owen the farmhand and Karen succumb to zombie bites and become undead.

After the loss of their father, the Doyle family – Leslie, Jonathan, and Jamie – move to the Fowler Mortuary in hope of starting a new life but find it in poor condition. Nomi Malone is a young drifter who hitchhikes to Las Vegas hoping to make it as a showgirl. After being cheated of her money by her driver, Nomi meets Molly Abrams, a seamstress and costume designer who takes her in as a roommate. Molly invites Nomi backstage at Goddess, the Stardust Casino show where she works, to meet Cristal Connors, the diva star of the topless dance revue. When Nomi tells Cristal she dances at Cheetah's Topless Club, Cristal derisively tells her that what she does is akin to prostitution. When Nomi is too upset to go to work that night, Molly takes her dancing at The Crave Club. After getting into a fight with James, a bouncer at the club, Nomi is arrested.
