The film begins with two anti-corruption detectives observing the discovery of dead bodies in a car being recovered from the bottom of a harbor. The detectives suspect that the drowned bodies are related to the recent gang war and power struggle at the huge Sanno-kai crime syndicate, which covers a large portion of eastern Japan. The new Grand Yakuza leader of Sanno-kai, Kato (Tomokazu Miura), must now reform his standing with the powerful rival Hanabishi-kai of western Japan. Otomo (Beat Takeshi), a former Yakuza, is serving time in a maximum security prison after falling out of favor with Sekiuchi, the former Grand Yakuza before Kato murdered him and took control.

The year is 1928 in Gyeong-seong (modern-day Seoul), Korea. Young Kim Jun-shik (Shin Sang-yeob), his father (Chun Ho-jin) and sister Eun-soo (Jo Min-ah) work on the farm of the Hasegawa family (Sano Shiro, Nakamura Kumi) in Japanese-occupied Korea. Both Jun-shik and young Hasegawa Tatsuo (Sung Yoo-bin) are interested in running; by the time they are teenagers (Do Ji-han, Kobayashi Yukichi), they have become fierce competitors. In the desert wilderness of 1930s Manchuria, Park Chang-yi, The Bad (Lee Byung-hun)—a bandit and hitman—is hired to acquire a treasure map from a Japanese official traveling by train. Before he can get it however, Yoon Tae-goo, The Weird (Song Kang-ho)—a thief—steals the map and is caught up in The Bad's derailment of the train. This involves the slaughter of the Japanese and Manchurian guards, and various civilians. Park Do-won, The Good (Jung Woo-sung)—an eagle-eyed bounty hunter—appears on the scene to claim the bounty on The Bad. Meanwhile, The Weird escapes, eluding his Good and Bad pursuers. A fourth force—a group of Manchurian bandits—also want the map to sell to the Ghost Market. The Weird hopes to uncover the map's secrets and recover what he believes is gold and riches buried by the Qing Dynasty just before the collapse of their government. As the story continues, an escalating battle for the map occurs, with bounties placed on heads and the Imperial Japanese Army racing to reclaim its map as it can apparently "save the Japanese Empire".

The events take place in 1932 in Japanese-occupied Manchuria, in which the corrupt leaders of the Japanese army are trying to take over all the Karate dojos /training halls for their own personal benefit. Amidst these circumstances the master Eiken Shibahara (portrayed by Yosuke Natsuki) from one of these dojo located on the southernmost Japanese island of Kyushu dies before passing on the Kuroobi/ black belt to his successor; leaving three of his pupils (Taikan (portrayed by Tatsuya Naka 7th Dan JKA Shotokan karate), Giryu (portrayed by Akihito Yagi 7th Dan Meibukan Gōjū-ryū Karate) and Choei (portrayed by Yuji Suzuki, 1st Dan Kyokushin karate) the task of deciding amongst themselves who deserves it most. After they bury their master, they are forced to leave the dojo and join the Japanese army. At this point, their journey leads them on rather different paths both in life and in the understanding of their master's teachings of martial arts, only to reunite them at the end, in a battle of corruption vs tradition.

Kitano plays Nishi, a violent and unpredictable police detective who quits the force after a terrible incident results in his partner, Horibe (Ren Osugi), needing to use a wheelchair.
