The film covers the court-martial of two U.S. Marines, Lance Corporal Harold Dawson and Private Louden Downey, who killed a fellow Marine, Private William Santiago, at the Guantanamo Bay Naval Base in Cuba. Santiago compared unfavorably to his fellow Marines, had poor relations with them, and failed to respect the chain of command in attempts at being transferred to another base. An argument evolves between base commander Colonel Nathan Jessup and his officers: while Jessup's executive officer, Lieutenant Colonel Matthew Markinson, advocates that Santiago be transferred immediately, Jessup regards this as akin to surrender and orders Santiago's commanding officer, Lieutenant Kendrick, to train Santiago to become a better Marine.

In 1965, Airman Second Class Adrian Cronauer (Williams) arrives in Saigon from Crete to work as a DJ for Armed Forces Radio Service. Cronauer is greeted by Private First Class Edward Montesquieu Garlick (Whitaker). Cronauer's irreverence contrasts sharply with many staff members and soon rouses the ire of two of his superiors, Second Lieutenant Steven Hauk (Kirby) and Sergeant Major Phillip Dickerson (Walsh). Hauk adheres to strict Army guidelines in terms of humor and music programming, while Dickerson is generally abusive to all enlisted men. However, Brigadier General Taylor (Willingham) and the other DJs quickly grow to like the new man and his brand of comedy. Cronauer's show consists of unpredictable humor segments mixed with news updates (vetted by the station censors) and rock and roll records that are frowned upon by his superiors.

Eleven-year-old Mark Sway and his little brother, Ricky, are smoking cigarettes in the woods near their home when they encounter Mob lawyer Jerome Clifford. Clifford tells Mark that he is about to kill himself to avoid being murdered by Barry "The Blade" Muldanno, the nephew of notorious mob kingpin Johnny Sulari. Ricky becomes catatonic after witnessing the suicide and is hospitalized. While driving cross-country from Boston to San Diego in their new Jeep Grand Cherokee, Jeff Taylor and his wife Amy narrowly miss colliding with a beat-up Ford F-150 that darts in front of them from a connecting road on a deserted highway. Later, while stopped at a gas station, the truck's driver, Earl, exchanges hostile words with Jeff before the couple resume their journey. Shortly afterwards, their Jeep breaks down in the middle of the desert. Leaving Jeff with the car, Amy accepts a ride from a passing big rig trucker named Red Barr driving a Peterbilt 385, to get to a nearby diner and call for help. Jeff eventually discovers that the Jeep's battery connections have been suspiciously tampered with, and, after reconnecting them, drives to the diner, only to discover that no one has seen his wife. When he catches up to Barr on the road and forces him to stop, the trucker claims he has never seen Jeff or his wife before. Jeff hails a passing sheriff, named Boyd, but a brief search of Barr's truck yields no sign of Amy, and he is let go.

Karl Childers (Billy Bob Thornton) is a mentally disabled Arkansas man who has been in the custody of the state mental hospital since the age of 12 for having killed his mother and her lover. Although thoroughly "institutionalized," Karl is deemed fit to be released into the outside world. Prior to his release, he is interviewed by a local college newspaper reporter, to whom he recounts the brutal murder of his mother and her boyfriend with a Kaiser blade - during which scene he notes to the reporter that, "Some folks call it a sling blade. I call it a kaiser blade," the line from which the film derives its name. Karl continues, saying that he killed the man because he thought he was raping his mother. When he discovered that his mother was a willing participant in the affair, he killed her too.
