The film begins by introducing Laurence (Melvil Poupaud), a 30-year-old man, who is an award-winning novelist and literature teacher in Montréal. Laurence is very much in love with her girlfriend, the fiery and passionate Fred (Suzanne Clément). On the day of her birthday, she reveals to Fred her biggest secret; she has felt for her entire life that she was born in the wrong body and says that she has been living a lie for so many years. She wishes to rectify her situation and restart her life as a woman. Fred accuses her of being gay and takes the news very hard. They separate for a short time, but Frédérique arrives at the conclusion, much to the chagrin of her mother and sister, that she must be there for Laurence. Their romance resumes and Frédérique becomes Laurence's biggest supporter. Frederique teaches Laurence how to do her makeup and buys her a wig. She urges Laurence to dress as her true self, in female clothing. Laurence shows up to work one day in a dress. All seemingly goes well until she is released from her position at the school due to the negative reception of her transformation. Fred falls into a state of depression and eventually leaves Laurence and moves away. Frederique marries another man, Albert, and has a son named Leo.

The movie is told through various points of view in a semi-documentary. Generosa (Poppy Montgomery) is a struggling artist who sells real estate, and one day, when client Ted (David Sutcliffe) fails to show up, she goes over to confront him. They soon fall in love, get married and are happy for several years during which time they adopt twin orphans: Alexa Ammon (Aislinn Paul) and Greg Ammon (Munro Chambers). Whether a result of being a mother or from an organic or drug-induced biochemical imbalance, Generosa becomes increasingly paranoid accusing Ted of, among other indiscretions, adultery. Her erratic behavior tears the marriage apart, thereby unleashing a very contentious divorce procedure with Generosa demanding custody of the twins, ownership of their house in the Hamptons and, if possible, all of Ted's wealth. Dr. Frank Linden has a life-size, anatomically correct medical dummy in his office which he calls "Pin". Via ventriloquism, Dr. Linden uses Pin to teach his children, Leon and Ursula about bodily functions and how the body works in a way the children can relate to without it being awkward. Dr. Linden's interactions with the children are otherwise cold and emotionally distant, and his ventriloquism act is the only sign of a more warm and playful side to his nature. Unknown to Dr. Linden, Leon is mentally ill and has come to believe that Pin is alive. Due in part to his mother, who discourages Leon from playing outdoors or bringing anyone home, Leon has no real friends and sees Pin as closest analogue. Leon is further traumatized when he secretly witnesses his father's nurse use Pin as a sex toy.
