Television executive Katy (Eva Mendes) develops what she hopes will become the most watched television show of all time, the ultimate game show called Live! where six contestants play Russian roulette with a revolver with six chambers loaded with one live cartridge and five dummy cartridges. During the show, after the cylinder is randomly turned, the six will fire the gun one by one pointed at their head, without intermediate extra turning of the cylinder, until one is killed. At first the idea is to give each of the six contestants $1,000,000, but while there are more applicants at the auditions than expected, they are suicidal people. This is not desired, the contestants should want to live; therefore the prize is raised to $5,000,000 each for the surviving contestants, while the family of the killed contestant does not get any prize.

Dr. Alice Dodgson (Jennifer Grey) is a failed physician that travels to Jamaica to care for Wesley Claybourne (Daniel Lapaine), a young man apparently suffering from encephalitis. Because she had been fired due to her actions resulting in the death of a patient, Alice has few options left and decides that she is willing to serve as Wesley's nurse. While in Jamaica Alice begins to fall for Wesley and starts to suspect that she and Wesley are the targets of a voodoo cult. She manages to befriend Caro (Kristen Wilson), a local girl that recommends that Alice not get overly involved with the rumors of voodoo and to overly mind her own business, since it's likely that if anyone is after Alice then it's because she is interfering. As things grow more tense, the viewers learn that Caro is behind all of the strange things going on and that she was seeking revenge due to Wesley's father killing her mother and denying that he fathered Caro, thus leaving her unable to inherit anything from him. Ultimately Caro managed to paralyze Alice and is about to turn her into a zombie, only for Alice to reveal that she was not completely paralyzed and causes Caro's plan to backfire, turning Caro into a zombie instead. Alice and Wesley then move back to the US and find jobs, whilst the local Police Chief takes the now zombie Caro home and puts her in his bed.

Mona Hibbard (Driver) is a young woman from a troubled home who has one overarching goal: to become the winner of the Miss American Miss pageant. Her mother is an alcoholic who graduates from berating her young daughter for not doing well in kids' pageants to declaring she will not provide any money or support for Mona if she keeps competing. Fortunately for Mona, she becomes best friends with Ruby Stilwell (Adams), who joins forces with her kind-hearted grandmother to help Mona as she begins her steady rise in the beauty pageant ranks. Unfortunately for Mona, she becomes pregnant and is horrified because women with children are not eligible for the MAM crown, and she has a young daughter (Eisenberg), but Ruby selflessly agrees to raise Vanessa as her own daughter. Mona becomes colder and meaner as she gets closer to achieving her goal, whether she's sabotaging a fellow contestant Leslie Stefanson on her routine and earning a lifelong enemy of an aspiring newscaster, or pawning all of her inconveniences onto Ruby while ignoring how obvious it is that Vanessa looks exactly like her, a path that leads to her victory in the Miss Illinois pageant. When Ruby is falsely accused of euthanasia and jailed, Mona is forced to take care of Vanessa, a task she is neither good at nor happy to do as she is afraid that her MAM crown bid will be ruined; Vanessa reacts to the situation by being angry and difficult to handle. A group of Russian mobsters have stolen a huge supply of paper for printing U.S. currency, and are now flooding the market with counterfeit bills. When one of the mobsters decides to turn herself in and hand over a data CD to the DA, she is shot and killed, but not before handing the disc to an unsuspecting Tommy Lee. Despite working with the police as a martial arts instructor, Lee doesn't go to the cops with the disc, but instead goes on the run, giving the mafia time to kidnap his daughter to hold as a hostage in exchange for the disc.
