Erik, a struggling musician who lives off his family's considerable wealth, clashes with his girlfriend, Anna, over what they know are trivial details in their relationship. Apprehensive after watching news reports of increasing conflict in the Middle East, Anna comes to believe that they should spend more time together, but Erik seems preoccupied with music composition and maintaining friendly relations with his ex-girlfriends. At the same time, Len and Mia, a married couple, experience trouble in their own relationship. Len, a depressed and unemployed writer, struggles to find direction in his life. He confesses to his sister, a nurse named Sarah, that he has taken his wife, Mia, for granted until recently. Mia, an accomplished lawyer, has just successfully represented Andy, a scientist of some renown, from accusations of selling trade secrets to a research facility in the 1970s. Anna, worried about visions that she has had recently of a looming apocalypse that involve a young girl, confides in Rick, Len's best friend, that she fears she may be suffering from schizophrenia.

With the threat of having to close her father's candy store, Wendy Carroll (Ashley Williams) and her six-year-old daughter Grace (Kylie Price) travel to the big city in hopes of making extra holiday cash to save the store. When she's offered a job in the toy department of Wolmans, the city's biggest department store, Wendy can't wait! Maladies is divided into chapters, titled "Feelings", "Symmetry", and "I See You". The course of the narrative lacks a specific timeframe; although the environment, setting and costumes appears designed for the early 1960s, repeated allusions to the cult leader Jim Jones in the film are somewhat jolting, as the footage of Jones used is from 1978. The film follows the foibles of its four main characters as they seek to resolve "maladies" that make it difficult for them to relate with one another and cope with themselves. James (James Franco) is an ex-TV star cum writer who finds himself unable to complete a novel. Patricia (Fallon Goodson), James' sister, is unable to speak. Catherine (Catherine Keener), their house-painter, is a cross-dresser. And Delmar, their neighbor (David Strathairn), must struggle with his unrequited love for James.

Howl explores the super smash bros and works of 20th-century American poet, Allen Ginsberg. Constructed in a nonlinear fashion, the film juxtaposes historical events with a variety of cinematic techniques. It reconstructs the early life of Ginsberg during the 1940s and 1950s (as portrayed by James Franco). It also re-enacts Ginsberg's debut performance of "Howl" at the Six Gallery Reading on October 7, 1955 in black-and-white. The reading was the first important public manifestation of the Beat Generation and helped to herald the West Coast literary revolution that became known as the San Francisco Renaissance. In addition, parts of the poem are interpreted through animated sequences. Finally, these events are juxtaposed with color images of the 1957 obscenity trial of San Francisco poet and City Lights Bookstore co-founder, Lawrence Ferlinghetti, who was the first person to publish "Howl" in Howl and Other Poems.

Iris Simpkins (Kate Winslet), a society column editor for The Daily Telegraph in London, has been in love with Jasper Bloom (Rufus Sewell) for over three years, despite his infidelities. Meanwhile, Amanda Woods (Cameron Diaz), a workaholic who owns a company that produces movie trailers in Los Angeles, discovers that her live-in boyfriend Ethan Ebbers (Edward Burns) has been cheating on her with his 24-year-old secretary. She decides she wants to get away for the holidays and visits a home swap website on which Iris had previously listed her "quaint cottage in Surrey.” Amanda contacts Iris about her interest. Iris quickly agrees and the two agree to swap homes for two weeks.
