Japon, vers 1900. Aveugle de naissance, la jeune Orin est abandonnée par sa mère à l'âge de six ans. Les habitants du village côtier d'Obama, dans la province de Wakasa la confient aux bons soins de Saito, le colporteur. Ce dernier emmène la fillette dans une maison de goze, une guilde de femmes aveugles, artistes itinérantes, soumises à des règles strictes de célibat. Orin grandit avec ces femmes, elle apprend le chant et le shamisen et devient à son tour une goze. Oyuki, a tattooed female assassin – the renegade member of a daimyo's personal bodyguard detail – is killing every man that is sent up against her. Fuyant son père et sa famille entière parce que ces derniers n'ont pas voulu accepter la relation amoureuse improbable qu'elle entretient avec un simple commis, Otsuya trouve refuge chez un ami, le dénommé Gonji. Ichi travels to the village of Itakura to pay respects at the grave of a man he killed two years earlier. At the grave he reminisces on a fight between the two yakuza gangs (Iioka and Sasagawa from an earlier film) one man ran away and, though Ichi was not taking part in the fight, attacked Ichi. It is 1860, and the era of the Tokugawa shogunate is coming to a close. A rōnin, or masterless samurai, wanders into a small town divided by a gang war between two gangsters, Seibei and Ushitora. Ushitora used to be Seibei's right-hand man, until Seibei decided that his son Yoichiro would succeed him. Tazaemon, the silk merchant and mayor, backs Seibei, while Tokuemon the sake brewer is allied with Ushitora. Gonji, the owner of an inn, advises the stranger to leave while he can, but after sizing up the situation, the rōnin tells Gonji that the town would be better off with both sides dead, and that he intends to stay.

Hekiro et Gen sont frère et sœur, ils s'entendent bien malgré un foyer peu chaleureux. Le père, un écrivain qui connait un certain succès, passe ses journées enfermé dans son bureau à travailler ses manuscrits. La belle-mère, une bigote chrétienne, est atteinte d'une forme sévère de rhumatismes qui la fait souffrir du pied et de la main gauche. Gen est une jeune fille au fort tempérament et responsable, elle supplée sa belle-mère handicapée dans les tâches quotidiennes et s'occupe avec affection de son frère Hekiro. Ce dernier, ne trouve pas sa place dans ce foyer et fait les 400 coups, entre bagarres au lycée, vol, dettes qu'il contracte dans une salle de billard ou chez un loueur de hors-bord. Cependant, malgré la confiance entre frère et sœur, certains problèmes surgissent. Quand Hekiro est atteint d'une tuberculose incurable, il s'attriste de voir sa sœur envisager de se marier. Le jour de sa mort, elle tâche de supporter courageusement le chagrin et la fatigue.
