Oh Mal-soon, a 74-year-old widow, lives with her son and his family. She is foul-mouthed, stubborn, controlling and shameless, and has a difficult relationship with her depressed daughter-in-law Ae-ja. She is proud of her son Hyun-chul, a university professor of gerontology, and the fact that she managed to raise him by herself against all odds. One day, Ae-ja gets hospitalized after suddenly collapsing, and doctors strongly advise her to live apart from Mal-soon. On the day she is told by her son that she is being sent to a nursing home, the dismayed Mal-soon wanders the streets and comes across a mysterious photo studio that claims it captures the moments of one's youth. Gang-cheol (whose name means "iron" in Korean) was once a legendary street gangster in Busan, but he put away his fists and cleaned up his act for the sake of his mother Soon-i, who has dementia. Things are going well for him; he now works as a stevedore at a loading dock and is pursuing a relationship with Soo-ji, a free-spirited woman from Seoul vacationing in the port city. But Soon-i is diagnosed with kidney failure and needs an expensive organ transplant that he can't afford. Knowing that Gang-cheol is desperate for money, local gang leader Sang-gon proposes that Gang-cheol come work for him and his brother Hwi-gon. At first Gang-cheol refuses, but when his debt-ridden best friend Jong-soo gives the deed to Gang-cheol's house to Sang-gon as collateral for a private loan, he's left with no choice but to get dragged back into Busan's criminal underworld. Gang-cheol is ordered to kill a visiting Japanese Yakuza boss whose death will enable Sang-gon's ascension within the Korean branch of the mob organization.

Jo-goo, a street magician (Lee Min-ki), notices a miserable looking girl, Yeo-ri (Son Ye-jin), in the audience during one of his performances, and she winds up being the inspiration for his “Horror magic show.” The show becomes a runaway success almost overnight and Jo-goo in turn, is now a successful stage magician with a model girlfriend. Yeo-ri works for Jo-goo, playing the specter in his show. Forward to almost a year later, the darkness inside her, something her colleagues sense, keeps her from truly connecting with them. When Jo-goo’s attempts to include Yeo-ri in a staff dinner finally succeeds, hilarious disaster ensues when she ends up blindingly drunk. Jo-goo calls Yeo-ri the next morning to sort things out, but when the phone disconnects with a weird sound, he decides to drop by her place for a visit instead. There, Jo-goo meets and engages in a game of hide-and-seek with a ghost child, which then scares the bejesus out of him later that night. As the two of them spend more time together, developing a fast friendship, Yeo-ri confesses the long story behind her gray, wan face.

Han-na is an overweight phone sex part-timer and a ghost singer for Ammy, a famous pop singer who actually lip syncs. Instead of being famous for her own amazing vocal talent, Han-na sings behind backstage during Ammy's performance and Ammy takes all credits, and records all of Ammy's songs. Other then that, Han-na has a huge crush for Sang-jun who's a director and his arrogant father owns the entertainment company. One day, Ammy purposely insults her in front of Sang-jun during his birthday party, knowing well that Han-na likes him. While crying in the bathroom, Han-na overhears Sang-jun telling Ammy that even though they are just using Han-na for her voice, they must be kind to her so she will not walk out on them. Heartbroken, Han-na attempts suicide with a gas leak but is interrupted by a phone call from one of her phone sex regulars who happens to be a top plastic surgeon. She tries to blackmail him with recorded tapes and later honestly tampers them to convince him about the pain she's holding in her heart about being fat. The surgeon refuses to operate further on Han-na, but Han-na threatens to inform his wife about his secret fascination. Then, Han-na makes a moving speech that she does not want to undergo surgery merely to be beautiful, but for the sake of love and as a vote of confidence so that she can live a happy life, and the surgeon is deeply moved in the end. Han-na puts herself in seclusion for a year as she recovers from the changes.

The film follows the lives of four childhood friends: Joon-seok, the leader of the group and whose father is a powerful mob boss; Dong-su, whose father is an undertaker; class clown Jung-ho; and Sang-taek, who was an exemplary student. As children they play together, sell sexually explicit pictures cut from a magazine, and wonder if a South Korean Olympic swimmer could outrace a sea turtle. Jung-ho also showed the rest of them a VCR that his mother had, and this film he found on it. It turns out to be a pornographic movie, and the boys are entranced while arguing over what a "menstruation" was, with them believing that it was the word that adults used to describe the "vagina".
