In 1996, Jamie Randall (Jake Gyllenhaal) is fired from a Pittsburgh electronics store for having sex with his manager's girlfriend. His wealthy brother Josh (Josh Gad) announces at the dinner table at their parents' (George Segal and Jill Clayburgh) house that he has found Jamie a job as a pharmaceutical sales representative. After attending a Pfizer training program where he has sex with the instructor (Kate Jennings Grant), Jamie goes to work for the company and tries to get doctors to prescribe Zoloft and Zithromax. He is rebuffed, much to the dismay of his regional manager, Bruce (Oliver Platt), who sees Jamie as his ticket to the "big leagues" of Chicago. Bruce says if Jamie can get Dr. Knight (Hank Azaria) to prescribe Zoloft instead of Prozac, other doctors will follow his lead. Jamie tries to get access to Dr. Knight by hitting on his female employees until, exasperated, Dr. Knight unethically permits him to observe him at work, during which time he accidentally sees a disrobing patient, Maggie Murdock (Anne Hathaway), who suffers from early onset Parkinson's disease.

Troy Gable (Colin Hanks) defies his father (Tom Hanks) and leaves law school to pursue his dream of becoming a writer in Los Angeles. To support himself, he takes a job as a road manager for "The Great" Buck Howard (John Malkovich), a fading mentalist. Troy comes to enjoy traveling with Buck to performances in smaller venues such as Bakersfield and Akron. On September 11, 2001, Port Authority Police officers John McLoughlin and Will Jimeno are patrolling the Port Authority Bus Terminal in Midtown Manhattan when they see a plane fly dangerously low overhead. As all of the police officers return to the station, they see on TV that the North Tower of the World Trade Center has been hit by the plane. Sergeant McLoughlin assigns many of the officers to assist in a precautionary evacuation attempt of the North Tower and they board a Metropolitan Transit Authority bus. On the bus, they hear reports that the South Tower is also hit by another plane. When they arrive at the World Trade Center, they realize the extent of the disaster, and see one of the victims jump out of the towers to certain death. The men proceed to get safety equipment from Building 5 and enter the concourse between the towers.

The film opens with Brad (Rory Cochrane), an out-of-work musician, making coffee for his wife, Lexi (Mary McCormack), who is still in bed. She leaves for work and Brad is left home alone. Gary (not his real name, which is never revealed) is a hustler walking through the streets of New York City, looking for business. On the way, he saves a young deaf transvestite from a group of gay bashers, but regrets it afterwards because the transvestite becomes infatuated with Gary and follows him everywhere. Gary introduces the transvestite to his friends: Valentino, a former porn star, and Mary Carmen, the Latina who works as a doughnut shop waitress and is in love with Valentino, with whom she has been living for some time. Together they form a bohemian family, which includes Veronica, a still-active porn star, and Nat, a tattoo artist. Gary is also in love with Valentino, who is dying of AIDS. Through the stages of the disease, Mary Carmen and Gary argue over what kind of care he should be receiving, and who is going to supply that care. As Valentino draws near death, Mary Carmen finds out she is carrying Valentino's baby. The three take stock of themselves and their relationships with one another.
