In the small town of Endora, Iowa, Gilbert Grape (Johnny Depp) is busy caring for Arnie (Leonardo DiCaprio), his brother with a developmental disability, as they wait for the many tourists' trailers to pass through town during their "yearly ritual" of camping at a nearby recreational area. His mother, Bonnie (Darlene Cates), gave up on life after her husband hanged himself seventeen years earlier. She spends almost all of her time on the couch watching TV. With Bonnie unable to care for her children on her own due to her morbid obesity, Gilbert has taken responsibility for repairing the old house and looking after Arnie, who has a habit of climbing the town water tower, while his sisters Amy and Ellen do the rest. The relationship between the brothers is of both care and protection, as Gilbert continually enforces the "nobody touches Arnie" policy. A new FoodLand supermarket has opened, threatening the small Lamson's Grocery where Gilbert works. In addition, Gilbert is having an affair with a married woman, Betty Carver (Mary Steenburgen).

Kurtwood Smith plays Myron Castleman, an everyman stuck in a loop that forces him to constantly relive the same hour of his life over and over, being the only person aware of this. During one loop, he discovers that a scientist named Nathan Rosenbluth has predicted an event that matches his experience. Castleman calls him and explains what is going on, however, Rosenbluth is highly skeptical of his claims. Over the next loops, Myron struggles to get into contact with Rosenbluth again, and in the process becomes frustrated to the point of screaming at his secretary and throwing his suitcase into traffic. Eventually, he does manage to talk to Rosenbluth, who initially dismisses Myron as crazy until Myron describes the transition as the scientist had predicted (including the phrase "Consciousness is an independent variable," which is central to Rosenbluth's theories). The professor sadly informs Castleman that there is nothing that can be done, causing Myron to become hysterical and shoot himself. There is a brief pause until Myron finds himself back at the beginning of the loop, realizing that he is trapped for eternity.

Hanna is facing a personal problem, as the police work is straining his relationship with his wife, Lillian (Pouget). As the Earth passes through the tail of a comet, previously inanimate objects (ranging from weapons to electric signs to electronics to vehicles to lawnmowers to an electric knife) start to show a murderous life of their own. In a pre-title scene, a man (King in a cameo) tries to withdraw money from an ATM, but it instead calls him an "asshole", and he whines to his wife (King's real life wife Tabitha). Chaos soon begins as machines of all kinds come to life and begin assaulting humans: a drawbridge inexplicably raises during heavy traffic, resulting in multiple accidents, most notably the black AC/DC van and a watermelon truck; while at a Little League game, a vending machine kills the coach by firing canned soda point-blank into his groin and then to his skull; a driverless steamroller flattens one of the fleeing children.

The film opens with 15-year-old Susan Bower on an alcohol and drug-fueled joyride with her boyfriend Ricky and two other friends, then intercuts to testimony from teenage and pre-teen addicts in a drug rehabilitation program run by Dr. Royce, who is then shown questioning the kids about the extent of their addictions. While foolishly attempting to drive with his eyes closed and both hands off the steering wheel, Ricky loses control of the car, causing it to skid on its side before overturning and landing all his passengers in the hospital (although ironically, Ricky himself emerges from the wreck unharmed). Once there, the presence of alcohol and drugs in Susan's bloodstream and urine is revealed, much to the shock of her parents (Helen and Frank, the latter a respected surgeon) and younger sister Kelly.
