Gilda, Olivia and Crocetta are three friends who live in a small island of Sicily. The women try to live as they can, being inept and provincial; but the chance of destiny soon arrive. In fact, the three women, one night, go to an island cave, because they suspect that Rocco, Olivia's husband, is going to meet an alleged lover there. Actually Rocco is a trafficker, who is trying to hide the loot in the cave, and so the three women, when they are discovered and threatened by man, Rocco is killed by gunshots. The three friends make the corpse disappear, and decide to keep quiet, but soon the police and criminals colleagues of Rocco put on their trail. In fact, men do not want so much about the death of Rocco, rather than looking for the loot that the women took.

The film begins in Sicily in 1940 during World War II just as Italy enters the war. A young boy, 12-year-old Renato, experiences three major events in one day: First, Italy goes to war; second, he gets a new bike; and third, he first sees the beautiful lady, Malena. Malena's husband, Nino Scordia, has been taken away to fight in Africa and Malena is left alone with her father, an elderly and almost-deaf man. Malena tries to cope with her loneliness, as the town she has moved to tries to deal with this beautiful woman who gets the attention of all the local men, including Renato. However, in spite of the gossip, she continues to be faithful to her husband. Renato becomes obsessed with Malena and starts fantasizing about her. His fantasies become increasingly elaborate and he becomes obsessed with the shy young woman, peeping in her window often as she waits sadly for her beloved husband to return. Renato eventually steals Malena's underwear and begins to fantasize about her in bed, to the horror of his parents. They do everything to stop his behavior, but it is all in vain.

The film opens with Peppino as a small child singing the popular song “Nel blu, dipinto di blu” with his brother in the back seat of a car on the way to a family gathering. The family is one of good standing in the social community and they are celebrating the fact that they have such a good life. In this scene the relationship between Peppino and his uncle Cesare is established. His uncle is a Don or Mafia boss in the small town of Cinisi where the story is set. In a scene soon after the happy family gathering we see don Cesare blown up by a car bomb which was planted by a rival Mafia boss. This ends Peppino’s time of innocence. Even as a small child he is thrust into the realities of life in the Mafia.
