The film begins in September 2008 (opening against the backdrop of news of the Islamabad Marriott Hotel bombing) with an elderly Lady Thatcher buying milk unrecognized by other customers and walking back from the shop alone. Over the course of three days, we see her struggle with dementia and with the lack of power that comes with old age, while looking back on defining moments of her personal and professional life, on which she reminisces with her (now-dead) husband, Denis Thatcher, whose death she is unable to fully accept. She is shown as having difficulty distinguishing between the past and present. A theme throughout the film is the personal price that Thatcher has paid for power. Denis is portrayed as somewhat ambivalent about his wife's rise to power, her son Mark lives in South Africa and is shown as having little contact with his mother, and Thatcher's relationship with her daughter Carol is at times strained.

Peter Colt (Paul Bettany), a British professional tennis player in his thirties whose ranking has slipped from 11th to 119th in the world, considers that he never really had to fight for anything as his wealthy, but not close, family easily put him through studies and allowed him to pursue his tennis ambitions. He bravely exchanges jokes with his German sparring partner Dieter Prohl (Nikolaj Coster-Waldau), who is in a similar position. Though he earns a wildcard spot to the Wimbledon tournament, he internally feels that it's time to admit he's getting too old to compete with fitter up and coming men (or boys) and intends, after this last Wimbledon, to take a job with the prestigious tennis club instead.

The film begins in the Elizabethan era shortly before the death of Queen Elizabeth I. On her deathbed, Elizabeth promises an androgynous young nobleman named Orlando a large tract of land and a castle built on it along with a generous monetary gift which she will only bequeath to him if he consents to her command, "Do not fade. Do not wither. Do not grow old." Both he and his heirs will keep the land and inheritance forever. Orlando acquiesces and resides in splendid isolation in the castle for a couple of centuries, during which time he dabbles in poetry and art. His attempts to befriend a celebrated poet, however, backfire when the poet ridicules his verse. Orlando then travels to Constantinople as British ambassador to the Turks, but he is almost killed in a diplomatic fracas there. Waking up the next morning, he learns something even more startling: he has physically transformed into a woman overnight.
