The film is about an evil living dead girl named Evelyn who decides that she needs friends. Her gifts for other girls (including a crow's heart) only scare them. Evelyn then tries to resurrect herself through suicide, after reading a book entitled How to Kill Yourself Back to Life. Her suicide attempts fail; part of the problem is that she wastes her bullets killing birds instead of herself, and she swings on her rope instead of hanging herself. Eventually, one boy (Close) is killed in a bus accident and becomes a living dead friend for her.

Luther falls in love with Hansel and the two decide to marry. This plan will allow Hansel to leave communist East Germany for the capitalist West. However, in order to be married, the couple must consist of a man and a woman. Hansel's mother, Hedwig, gives her child her name and passport and finds a doctor to perform a sex change. The operation is botched, however, leaving Hansel – now Hedwig – with a dysfunctional one-inch mound of flesh between her legs, the eponymous "Angry Inch".

Jane Doe (Hatcher) is the real name of arms manufacturer Cy-Kor's recently fired security password employee with top clearance, whose teenage son Michael (Trevor Blumas) is kidnapped. She obeys the bizarre instructions, including getting and learning to use a gun and downloading a secret file (after which her work post starts totally deleting), dumping both in a dumpster and waiting nearby, only to witness the company's CEO Churnings being shot by a sniper using an identical weapon. Michael is released, but the pair is now wanted for the murder and both are abducted by armed men, who bring them to a ranch. There they are welcomed by Michael's father, David Doe (Lowe), who discloses to be an agent of the Defense Intelligence Agency (DIA), and so is Michael, who used her clearance as the whole thing is a sting for the kidnapping and murder's brain, Avery (Mark Caven) so he will sell the enemy false data. Alas David's DIA-partner Kurt Simmons and their boss Phelps have a dirty agenda.

A man named Alderson awakens in a cube-shaped room with a hatch in each wall, the ceiling and the floor, each of which leads to other cube-shaped rooms, identical except for their color. He enters an orange room and, without warning, is killed by a trap. In another such room, five people – Quentin, Worth, Holloway, Rennes, and Leaven – meet. None of them knows where they are or how they got there. Quentin informs the others that some rooms contain traps, which he learned by nearly being killed by one. Rennes assumes each trap is triggered by a motion detector and tests each room by throwing one of his boots in first. Leaven notices numbers inscribed in the passageways between rooms. Quentin, a policeman, recognizes Rennes as "the Wren", an escape artist renowned for getting out of jails. After declaring one room trap-free, Rennes enters and is killed when he is sprayed with acid. The others realize that there are different kinds of detectors, and Quentin deduces that this trap was triggered by heat.
