In 1945 a young couple, Ivor Hartley (Daniel Maslany) and Irene (Tracy-Lynn Pugsley) are leaving the homecoming dance. The couple take a shortcut through the woods where they begin to make out. Irene wants to stop, but Ivor attempts to rape her. She manages to injure him, however, so he leaves. As Irene makes her way out of the shortcut, Benjamin Hartley attacks her, hitting her over the head with a rock and killing her.

During a shootout against Chinese Triads at a San Francisco dock warehouse, FBI agents John Crawford (Jason Statham) and Tom Lone (Terry Chen) stumble across the notorious assassin Rogue (Jet Li), a former CIA assassin who now works for the Japanese Yakuza. Rogue ambushes Crawford and is about to execute him when Lone appears and shoots Rogue in the face, causing him to fall into the water. Rogue's body was never found and he is presumed dead. However, Rogue survives and retaliates against Lone, his wife and his daughter. He kills them, burns down the house, and leaves their three corpses in the ashes of their home.

Jonathan Rivers (Michael Keaton) is a successful architect and lives a peaceful life with his wife Anna (Chandra West) until her unexpected disappearance. Eventually, he is contacted by Raymond Price (Ian McNeice), who claims that his own son had also died. He says he has recorded messages from Anna through electronic voice phenomena (EVP). While Jonathan is initially dismissive and angered, he later learns about his wife's tragic drowning. Desperate, he begins to believe that the recorded voice is indeed that of his wife. Jonathan becomes obsessed with trying to contact her himself, despite warnings from a psychic, Mirabelle Keegan (Keegan Connor Tracy), who tries to tell him how the recording can attract other, unwanted entities. A woman named Sarah Tate (Deborah Kara Unger), who also came to Raymond for his EVP work because she lost her fiancé, befriends Jonathan.
