In 1927, silent film star George Valentin is posing for pictures outside the premiere of his latest hit film when a young woman, Peppy Miller, accidentally bumps into him. Valentin reacts with humor to the accident and shows off with Peppy for the cameras. The next day, Peppy finds herself on the front page of Variety with the headline "Who's That Girl?" Later, Peppy auditions as a dancer and is spotted by Valentin, who insists that she have a part in Kinograph Studios' next production, despite objections from the studio boss, Al Zimmer. While performing a scene in which they dance together, Valentin and Peppy show great chemistry, despite her being merely an extra. With a little guidance from Valentin (he draws a beauty spot on her, which will eventually be her trademark, after finding her in his dressing room), Peppy slowly rises through the industry, earning more prominent starring roles.

The film starts with a terrified mother and her young son. As she packs a suitcase, she hears something coming towards the door and makes her son hide under the bed. The door bursts open and a supernatural force throws the mother against the wall, killing her. The son flees and runs into his terrified sister. The boy hides under a table, but his sister is caught and pushed over the banister of the stairs, then dragged down to the cellar. The boy hides but is eventually found by the attacker.

In 1969, Dr. Malcolm Sayer (Robin Williams) is a dedicated and caring physician at a local hospital in the New York City borough of The Bronx. After working extensively with the catatonic patients who survived the 1917–1928 epidemic of encephalitis lethargica, Sayer discovers certain stimuli will reach beyond the patients' respective catatonic states; actions such as catching a ball, hearing familiar music, and experiencing human touch all have unique effects on particular patients and offer a glimpse into their worlds. Leonard Lowe (Robert De Niro) proves elusive in this regard, but Sayer soon discovers that Leonard is able to communicate with him by using a Ouija board.

John Whitney (Lewis Van Bergen), an anthropologist for the Museum of Natural History in Chicago, is studying a newly discovered tribe in South America, and drinks a soup made for him by the tribesmen. A short time later, he is seen accosting the captain of a merchant ship, asking that the shipment he has sent back to Chicago be unloaded from the ship. When the captain informs him that it's too late because the ship is about to leave, Whitney sneaks aboard. As the ship leaves the dock, Whitney searches through the cargo hold for his crates, crying out in despair when he does not find what he's looking for.
