Two years after the Battle of New York, Steve Rogers works in Washington D.C. for the espionage agency S.H.I.E.L.D. under Director Nick Fury, while adjusting to contemporary society. Rogers and Agent Natasha Romanoff are sent with S.H.I.E.L.D.'s counter-terrorism S.T.R.I.K.E. team, led by Agent Rumlow, to free hostages aboard a S.H.I.E.L.D. vessel from Georges Batroc and his mercenaries. Mid-mission, Rogers discovers Romanoff has another agenda: to extract data from the ship's computers for Fury. Rogers returns to the Triskelion, S.H.I.E.L.D.'s headquarters, to confront Fury and is briefed about Project Insight: three Helicarriers linked to spy satellites, designed to preemptively eliminate threats. Unable to decrypt the data recovered by Romanoff, Fury becomes suspicious about Insight and asks senior S.H.I.E.L.D. official Alexander Pierce to delay the project.

In 1954, two U.S. Marshals - Edward "Teddy" Daniels and his new partner, Chuck Aule - travel to the Ashecliffe Hospital for the criminally insane on Shutter Island located in Boston Harbor as part of an investigation into the disappearance of patient Rachel Solando, incarcerated for drowning her three children. The sole clue is a note left by Solando, which reads: "The law of 4; who is 67?" Shortly after arrival, a storm prevents their return to the mainland for several days. Daniels finds the staff confrontational: the lead psychiatrist, Dr. John Cawley, refuses to hand over records of the hospital staff; Solando's doctor, Dr. Lester Sheehan, had left on vacation after her disappearance, and they are barred from searching Ward C and told that the lighthouse on the island has already been searched, so there is no need to search it. When a patient is being interrogated by Daniels, with a subterfuge she sends Aule away for a few seconds and scribbles "RUN" in Daniels' notepad.

U.S. energy giant Connex is losing control of key Middle East oil fields in a kingdom ruled by the al-Subaai family. The emirate's foreign minister, Prince Nasir (Alexander Siddig), has granted natural gas drilling rights to a Chinese company, greatly upsetting the U.S. oil industry and government. To compensate for its decreased production capacity, Connex initiates a shady merger with Killen, a smaller oil company that recently won the drilling rights to key petroleum fields in Kazakhstan. Connex-Killen ranks as the world's twenty-third largest economy, and antitrust regulators at the U.S. Justice Department (DOJ) have misgivings. A Washington, D.C.-based law firm headed by Dean Whiting (Christopher Plummer) is hired to smooth the way for the merger. Bennett Holiday (Jeffrey Wright) is assigned to promote the impression of due diligence to the DOJ, deflecting any allegations of corruption.

At his son's wedding party, Edward Bloom (Albert Finney) tells the same tall tale he's told many times over the years: on the day Will (Billy Crudup) was born, he was out catching an enormous uncatchable fish, using his wedding ring as bait. But when his father's health starts to fail from cancer, Will and the now pregnant Joséphine return to his hometown in Alabama to visit. On the plane, Will recalls his father's tale of how he braved a swamp as a child after he was dared by a few other children. He meets a witch (Helena Bonham Carter). She shows Don Price and another boy how they were going to die. They run away, frightened. When the witch shows Edward his death in her glass eye, he accepts it without fear. With this knowledge, Edward knew there were no odds he could not face.
