Priyamani (born: June 4, 1984, Bengaluru) is an Indian multilingual film actress who works in Kannada, Hindi, Tamil, Telugu and Malayalam Cinema. Priyamani gained widespread recognition for her role as village girl Muththazhagu in the Tamil film Paruthiveeran in 2007, receiving National Film Award for Best Actress and Tamil Nadu State Film Award for Best Actress for her performance. During the same year, she achieved commercial success in Telugu cinema with S. S. Rajamouli-directed Yamadonga. In 2008, Priyamani received further critical acclaim for her role as Malavika in the Malayalam film Thirakkatha, written and directed by Ranjith. The following year, she made her Kannada film debut in Raam which proved to be a commercial and critical success. Priyamani made her Hindi film debut through Mani Ratnam's bilingual in Tamil and Hindi, titled Raavan and Raavanan respectively. In 2012, her portrayal of conjoined twins in the multilingual film Chaarulatha, based on Thai film Alone, gained critical praise.

Rahul Mithaiwala (Shahrukh Khan) is a forty-year-old bachelor living in Mumbai. His parents died in a car accident when he was eight years old, so his grandparents raised him. His grandfather (Lekh Tandon) owns a chain of confectionery shops where Rahul has been a dedicated worker helping his grandfather. Two of Rahul's friends convince to finally take a vacation and travel with them to Goa. Rahul's grandfather dies on the day of the 100th birthday celebration. His grandmother tells Rahul that his grandfather wanted his ashes to be divided into two parts—one of each to be immersed in the Ganges and Rameswaram. She asks Rahul to take the ashes to Rameswaram and disperse them. While he accepts her request, Rahul and his friends plan to dump the ashes at Goa but his grandmother wants to see him off, forcing Rahul to travel by train. He books a single ticket on the Chennai Express, planning to quickly exit and meet his friends along the way. However, as he tries to leave the train, he notices a young woman running to board it. He helps her and then four large men board the moving train, missing his chance to debark.

Chandrasekhar (Mohanlal) is made the head of the Metro Crime Stopper Cell in Kochi. The story takes it off showing the conjoined twins Charu and Latha (both played by Priyamani). Charu and Latha lived in Vizag as one body and soul until Ravi (Skanda) comes into their lives. Ravi falls in love with one of the twins which creates differences between the two. Finally, their mother (Saranya) plans to separate their attached bodies through surgery and during which Latha dies. However, mysterious incidents take place as the dead twin seeks revenge on the other. Several twists take place in line as the story unfolds. She also sees the dead twin playing the violin they used to play in the past and the twin starts to come forward to reach her and disappears as Ravi touches Charu. Later, in the hospital, Ravi visits the twins' mother (Saranya) where she tries to tell Ravi something but is unable to do so. As the problems began to go down, Ravi again visits the twins' mother and she tells him something at the graveyard. Ravi goes to the cemetery, only to find that the real person who died was Charu and that Latha was impersonating Charu. Flashbacks reveal that after falling in love with Charu, Ravi comes to visit Charu finally as he was leaving town which makes Latha angry. Latha refuses to follow Charu to the balcony to see Ravi for the last time. Ravi leaves after giving a paper to the guard containing a picture of Charu, delighting Charu but leaves Latha outraged. Latha tears the picture angrily and this creates problem between the twins. Latha fights with Charu on the stairs and both of them fall from the stairs. In this, Charu dies and Latha calls her mother to help. After this, both of them are separated. In reality, after fighting with Ravi, she ties him up in chair and tells him the truth that she also loved him. Later, Charu burns the place to make Ravi escape and Latha stays inside there, refusing to leave unless Ravi tells that he, too, loves her. Latha's mother scolds and begs Ravi to save her only daughter. Ravi sacrifices his love to save Latha. The film ends with Ravi and Latha keeping flowers for Charu and Ravi sees Charu's soul smiling at him as Latha walks towards him.

The film opens with the history of a village called Kshetram. An idol of Lakshmi Narasimha Swamy from the temple is hidden in a forest in order to save it from invaders. Generations of the Rayulu family have struggled to reinstate the idol in the temple but were unable to do so. After few generations, Veera Narasimha Rayalu (Jagapati Babu) a powerful village leader of same dynasty want to finish the ancient ritual, which is critical, to the well being of his village. Veera Narasimha Rayalu changes his name to Naga Penchalayya being the process of the ritual, but he has been killed by his own uncle (Kota Srinivasa Rao) and his cousin brother Viswanadha Rayulu (Adithya Menon) to conquer his position and property. The ritual remains incomplete So, his wife Lakshmi (Priyamani) decides to continue the process, changes herself as Naga Penchalamma to complete the ritual as her husband's last wish. Naga Penchalamma and her entire family also killed by same baddies. Before dying, she takes an oath that in any circumstances she will come back to succeed the machine. After 25 years Chakradeva Rayalu / Chakri (Kick Shyam) is the son of Viswanadha Rayulu falls in love with Sohini Agrawal (again Priyamani), who is eerily similar to Lakshmi. When Sohini comes into the ancestral house of Chakri, strange supernatural events happen and powerful spirits and athmas come into the picture. What happens to Sohini? Will the ritual be completed? That is the story.
