While driving cross-country from Boston to San Diego in their new Jeep Grand Cherokee, Jeff Taylor and his wife Amy narrowly miss colliding with a beat-up Ford F-150 that darts in front of them from a connecting road on a deserted highway. Later, while stopped at a gas station, the truck's driver, Earl, exchanges hostile words with Jeff before the couple resume their journey. Shortly afterwards, their Jeep breaks down in the middle of the desert. Leaving Jeff with the car, Amy accepts a ride from a passing big rig trucker named Red Barr driving a Peterbilt 385, to get to a nearby diner and call for help. Jeff eventually discovers that the Jeep's battery connections have been suspiciously tampered with, and, after reconnecting them, drives to the diner, only to discover that no one has seen his wife. When he catches up to Barr on the road and forces him to stop, the trucker claims he has never seen Jeff or his wife before. Jeff hails a passing sheriff, named Boyd, but a brief search of Barr's truck yields no sign of Amy, and he is let go.

The film begins with Jack Ruby saying, "Lookin' back at it now, what can you say? It feels like it was a dream. Yeah, that's it, a dream. Maybe none of it never happened. Because when I look back on it today, this is the best sense I can make of it." Then the scene shows a murder; a corpse, dressed in a suit, is being drained of blood, having been hung on a meat hook. It is readily apparent that the corpse has been tortured, and it is implied that the presentation of the body is intended to be a brutal message.

They end up going to the Jupiter Hollow Hospital but the doctor will not check them in because it is exclusively for employees of Hollowmade, the local furniture maker. Mr. Shelton buys the company on the spot. The Ratliffs, an impoverished couple, arrive moments later with Mrs. Ratliff also in labor. Both women give birth to twin girls. The doctor tends both births at the same time, and the elderly nurse gets confused and mixes the babies. Dr. Scott McKenzie (William Devane) is a college professor and ardent fan of the gunslinger culture of the Old West. A year ago, McKenzie watched his wife and son die in a car crash caused by a drunk driver attempting to flee the police. With his friend, General Joe Brodsky (John Ratzenberger), McKenzie attends an auction of Wild West memorabilia, where they agree to bid on a pair of steamer trunks and split the contents between them. As the auction is conducted, flashbacks show the items' histories in the 19th century. At the time a man, Joseph Cole (Klaus Kinski), is looking for a gunslinger who has a pair of distinctive ebony-handled pistols marked with stars. Some cowboys he encounters on the road point him towards the town of Crossfire, California. At the town saloon, he inquires about the man but is harassed by a trio of local thugs, one of whom shoots at him, hitting instead one of the trunks which Dr. McKenzie is bidding on in the present. Cole quickly shoots all three men dead, an event memorialized by a local photographer. In the present, McKenzie and Brodsky win the trunks at auction and McKenzie begins sorting through the contents, noticing the picture of the men who Cole killed.

Jake Farris (Dolly Parton), a country singer stuck in a long-term contract performing at a sleazy urban cowboy nightclub in New York City, boasts to the club's manager, Freddie (Ron Leibman), that she can make anybody into a country sensation, insisting that she can turn any normal guy into a country singer in just two weeks. Freddie accepts Jake's bet, putting up the remainder of Jake's contract (if she wins the bet, the contract becomes void; if she loses, another five years will be added). He then ups the ante: if Jake loses, she must also sleep with him. The problem is that Freddie can select the man, and he selects an obnoxious New York cabbie named Nick Martinelli (Sylvester Stallone). Nick not only has no musical talent whatsoever, he claims to hate country music "worse than liver". Realizing she is stuck with Nick, Jake takes him back to the hills of Tennessee for a two-week crash course in how to walk, talk, and sing like a genuine country star.
