The film begins with an opening quote from James 2:17. A pastor (McGinley) is shaken by a street preacher regarding his faith, which causes a chain of events for twelve different people, moving towards one cataclysmic event. Some will survive while others will not, but many lives will be changed. An older couple (Shepherd and Majors) have lost their only child. The aforementioned pastor and his wife (Tracy Melchior) were unable to conceive a child. A mother (Sorvino) and young daughter (Makenzie Moss) find themselves homeless, a young nurse (Dominguez) and her husband (Matthews) are caught in a court case that may ruin their lives. A young mother (Pettis) is about to give birth. A war veteran (Soria) returns home with PTSD. A suicidal young woman (PenaVega) seeks to see her father and feel loved again. A street gang is caught up in the mix when one of the members (Shwayze) unexpectedly ends up in church while running and hiding from the police.

The Expendables, a group of elite mercenaries based in New Orleans, deploy to the Gulf of Aden to save hostages on a vessel from Somali pirates. The team consists of leader Barney Ross, blades specialist Lee Christmas, martial artist Yin Yang, Gunner Jensen, weapons specialist Hale Caesar, and demolitions expert Toll Road. Jensen instigates a firefight, causing casualties for the pirates. He then tries to hang a pirate, but Yang stops him when Ross and the team discourage it. Ross reluctantly discharges him from the team. Later, Christmas is upset to discover his girlfriend, Lacy, has left him for another man.

The film focuses on Second Lieutenant Bart Gregory (David Anders) who has been killed under mysterious circumstances in Iraq. After his friends and girlfriend Janet (Louise Griffiths) attend his funeral, Bart awakens in his grave. Enlisting the help of his best friend, Joey Leubner (Chris Wylde), Bart begins to understand and learn how to deal with his new undead state; mainly, the fact that Bart needs blood to hold back decomposition and that he returns to a state of in-animation during daylight hours. Joey does research online to find out what Bart is and seems to be stuck between a Zombie and Vampire, finally stating that Bart is a Revenant.
