In 1929, Italian dictator Benito Mussolini (Rod Steiger) is still faced with the 20-year-long war waged by patriots in the Italian colony of Libya to combat Italian colonization and the establishment of "The Fourth Shore"—the rebirth of a Roman Empire in Africa. Mussolini appoints General Rodolfo Graziani (Oliver Reed) as his sixth governor to Libya, confident that the eminently accredited soldier and fascist Grande can crush the rebellion and restore the dissipated glories of Imperial Rome. Omar Mukhtar (Anthony Quinn) leads the resistance to the fascists. A teacher by profession, guerrilla by obligation, Mukhtar had committed himself to a war that cannot be won in his own lifetime. Graziani controls Libya with the might of the Italian Army. Tanks and aircraft are used in the desert for the first time. The Italians also committed atrocities: killing of prisoners of war, destruction of crops, and imprisoning populations in concentration camps behind barbed wire.

When the existence of a strain of plague (vaguely identified as pneumonic) is revealed at the U.S. mission at the International Health Organization, three terrorists seek to blow up the U.S. mission. Two of them are shot, one mortally, by security personnel but one escapes. The surviving terrorist is hospitalized and quarantined and identified as Swedish. Dr. Elena Stradner (Ingrid Thulin) and U.S. Colonel Stephen Mackenzie (Burt Lancaster) (Military Intelligence assigned to the IHO) argue over the nature of the strain, which Stradner suspects is a biological weapon but which Colonel Mackenzie claims was in the process of being destroyed.

Retired American professor (Burt Lancaster) lives a solitary life in a luxurious palazzo in Rome. Against the backdrop of the Bourbon Restoration, Lombard aristocrat Fulvio Imbriani, a former political extremist who once served under Napoleon, is finally released from an Austrian jail, after a lengthy sentence for his part in the secret Sublime Brotherhood. He strives to resume normal family life, but his Hungarian lover, Charlotte, together with his ex-comrades, succeeds in convincing him to take part in a revolutionary cause in the south. In fact, Fulvio considers the effort futile and fails to stop his sister Esther from reporting the conspirators. But the trap fails to catch the intended prey and, evading arrest, the comrades meet and bury Charlotte, who was killed in an exchange of fire with the gendarmerie.

Évocation de la vie de Louis II de Bavière, depuis son couronnement à l'âge de dix-huit ans et demi jusqu'à son internement et sa mort à quarante. On y découvre la complicité presque amoureuse qui le lie à sa cousine Sissi (la jeune impératrice d'Autriche-Hongrie) qui est à deux doigts de parvenir à lui faire épouser sa sœur Sophie malgré le peu d'attirance qu'il a pour celle-ci, son entichement déraisonnable pour la musique de Richard Wagner, dont il devient le très généreux mécène au point de lui faire construire un opéra, les circonstances qui l'amènent à céder aux penchants qui lui seront funestes : son goût du rêve, du post-romantisme, des garçons (son palefrenier devenant son chambellan et homme de confiance très intime), des châteaux de contes de fées, pour l'édification desquels il dépense des fortunes et dans lesquels il fuira les dures réalités de son temps (à savoir : l'irrésistible unification allemande autour de la Prusse de Bismarck qui vassalise tous les autres royaumes ou principautés germaniques, Bavière y compris) en s'imaginant, entouré d'une garde rapprochée de serviteurs, qu'il est encore vrai roi en son royaume. Jusqu'à ce que le gouvernement effectif de Munich l'extirpe de son rêve et l'interne au château de Berg… où il meurt dès le lendemain de son arrivée en tentant, dans des circonstances jamais vraiment élucidées, de s'évader.
