The film begins in Hudson, Ohio, where Scott "Scotty" Thomas (Scott Mechlowicz) gets dumped by his girlfriend, Fiona (Kristin Kreuk), immediately after his high school graduation in 2004. With his best friend, Cooper Harris (Jacob Pitts), Scott attends a graduation party that evening and listens to a performance of the film's main theme song "Scotty Doesn't Know", that crudely details the affair Fiona was having with the band's singer, Donny (Matt Damon), while she was with Scott. Drunk and angry, Scott returns home and receives an e-mail reply from his German pen pal, Mieke (Jessica Boehrs), who expresses sympathy towards Scott after Fiona breaks up with him, and suggests they arrange to meet in person. However, the entire time Scott has known her he has mispronounced Mieke as "Mike", thinking that she is male. After Cooper suggests that "Mike" may be a sexual predator, Scott angrily tells Mieke to stay away from him. Scott's younger brother, Bert (Nial Iskhakov), informs him that "Mieke" is a common girl's name among Germans. Realizing his mistake, and that he has feelings for Mieke, Scott tries desperately to contact her again, only to find out that Mieke has blocked his email address. Encouraged by Cooper and with him in tow, Scott decides to travel to Europe, seek out Mieke, and apologize to her face-to-face.

After rescuing U.S. Marshal Ray Carter (Robert Patrick) in Mongolia, the Angels, Dylan (Drew Barrymore), Natalie (Cameron Diaz), and Alex (Lucy Liu) are hired to locate a set of titanium rings called H.A.L.O. (Hidden Alias List Operation) stolen from the Department of Justice that, when put together in a machine, will display a list of all individuals in the witness protection program. Many people, including Justice Department official William Rose Bailey (Bruce Willis), who possessed one of the rings, were murdered in the process. Having discovered that one of the protected witnesses, Alan Caulfield (Eric Bogosian), was murdered in his house in San Bernardino, the Angels investigate the scene and discover evidence suggesting that Caulfield was smothered with a bag and the killer is a surfer with a scar on his left leg. Meanwhile, Natalie and her now boyfriend Peter Kominsky (Luke Wilson) are moving in together, with some help from Alex's boyfriend Jason Gibbons (Matt LeBlanc), whose relationship with Alex is on a "time out".

Jim Evers is a workaholic real estate agent whose continuous pursuit for new deals leaves very little time with his family. Danyael Rosales is a street preacher who thinks God does not care about anyone because of the death of his parents, Valerie Rosales and the angel Danyael from the previous film. He is then forced to face his destiny. As a Nephilim, he has some of the angels' abilities, such as regeneration, and can only be killed if his heart is removed. One night, a blind assassin shoots Danyael as he preaches before a crowd, but the assassin is driven off before he can take out Danyael's heart. As punishment for his failure, Zophael kills the assassin and goes after Danyael himself with an extendable weapon with a blade that can be turned into a three-pronged hook. However, Danyael is protected by Gabriel, a now-human fallen angel who killed Danyael's father and performed many misdeeds. After being defeated by Danyael's mother, Gabriel was turned into a human as punishment. Having spent years as a human, he now realizes how wrong he was in the past.

Thomas Dagget, a seminary student, loses his faith when he is shown disturbing visions of a war between angels. Years later, Thomas is a detective with the Los Angeles Police Department.
