On October 31, 1989, Michael Myers and his niece Jamie Lloyd (Danielle Harris) have been abducted by a mysterious stranger in black at the Haddonfield Police Station. Six years later, teenager Jamie (J. C. Brandy) has been impregnated and her baby is born on Halloween Eve. The baby is taken away by the "Man in Black" who appears to be the leader of a Druid-like cult. Later, a midwife (Susan Swift) helps Jamie escape with her baby and is soon killed by Michael (George P. Wilbur). Jamie and her baby flee in a stolen pickup. Stopping briefly at a deserted bus station, Jamie makes a call to a Haddonfield radio station to warn them that Michael is about to return home, only to be ignored by the radio D.J. Barry Simms (Leo Geter) who is doing a broadcast on the Haddonfield murders.

Gary Tison (Robert Mitchum) is serving two consecutive life sentences for murder, while his three young adolescent sons try to move on with their life. The oldest, Donny (James Spader) is the only son making something of his life. He is a pre-law student and is in a loving relationship with his high school sweetheart Carol (Catherine Mary Stewart). His younger brothers Ray (Lance Kerwin) and Ricky (Eric Stoltz) spend their days doing nothing and decide - convinced that he is innocent - to break their father out of jail. Donny, morally challenged, threatens to give them up the police. Noticing their determination, he agrees to go along, convinced that they can never break Gary out without his help. The three young men drive up to prison and Donny and Ricky use their guns to keep everyone hostage, while Ray breaks out Gary and his cellmate Randy Greenawalt (Stuart Margolin).

Stella Johnson (Barbara Eden) is a beautiful widowed single mother who lives in the town of Harper Valley, Ohio. She sells cosmetics door-to-door for the fictitious AngelGlo Cosmetics (akin to the real-life Avon). Her fourteen-year-old daughter is named Dee (Susan Swift), a student at Harper Valley Junior High School. Ivy Templeton (Susan Swift) is a ten-year-old girl, living with her parents, Janice and Bill Templeton (Marsha Mason and John Beck), in New York City. Her parents notice a stranger stalking them over the course of a few weeks, and discover, over lunch with him, that his name is Elliot Hoover (Anthony Hopkins). Hoover is convinced that Ivy is a reincarnation of his daughter Audrey Rose, who died in a fiery car accident, along with his wife, two minutes before Ivy was born. Hoover had come to believe this through information given to him by two clairvoyant psychics. Bill asks a friend of his, an attorney, to hide in their apartment to hear Hoover's full story to build a case against him, but when Hoover speaks Audrey's name out loud, Ivy hears him from her room and enters an altered state where she cannot be calmed down without the assistance of Hoover. In this state, she bangs her hands on a window and becomes burned, which Hoover says is a result of his daughter's experience of being burned alive in the car.
