In 1954, two U.S. Marshals - Edward "Teddy" Daniels and his new partner, Chuck Aule - travel to the Ashecliffe Hospital for the criminally insane on Shutter Island located in Boston Harbor as part of an investigation into the disappearance of patient Rachel Solando, incarcerated for drowning her three children. The sole clue is a note left by Solando, which reads: "The law of 4; who is 67?" Shortly after arrival, a storm prevents their return to the mainland for several days. Daniels finds the staff confrontational: the lead psychiatrist, Dr. John Cawley, refuses to hand over records of the hospital staff; Solando's doctor, Dr. Lester Sheehan, had left on vacation after her disappearance, and they are barred from searching Ward C and told that the lighthouse on the island has already been searched, so there is no need to search it. When a patient is being interrogated by Daniels, with a subterfuge she sends Aule away for a few seconds and scribbles "RUN" in Daniels' notepad.

In Houston, 1913, nine-year-old Howard Hughes is warned by his mother of the diseases to which she is afraid he will succumb. Fourteen years later, he begins to direct the movie Hell’s Angels. However, after the release of The Jazz Singer, the first partially talking film, Hughes becomes obsessed with shooting his film realistically, and decides to convert the movie to a sound film. Despite the film being a hit, Hughes remains unsatisfied with the end result and orders the film to be re-cut after its Hollywood premiere. He becomes romantically involved with actress Katharine Hepburn, who helps to ease the symptoms of his worsening obsessive-compulsive disorder (OCD).

On February 6, 1846, at Paradise Square in Lower Manhattan's Five Points, a territorial battle of hand-to-hand combat between Bill "the Butcher" Cutting's U.S.-born nativist gang, the Natives, and "Priest" Vallon's Irish Catholic immigrant gang, the Dead Rabbits, concludes when Cutting kills Vallon, witnessed by Vallon's young son, Amsterdam. Cutting declares the Dead Rabbits outlawed but orders that Vallon's body be buried with honor. Sam Bowden (Nick Nolte) is a lawyer in the quiet town of New Essex, North Carolina. Max Cady (Robert De Niro) is a former client whom Bowden defended 14 years earlier when he was working as a public defender in Atlanta. Cady was being tried for the violent rape and battery of a young woman. Bowden, appalled by Cady's crime, buried crucial evidence about the victim—that she was promiscuous—which might have lightened Cady's sentence or even secured his acquittal, violating his ethical and professional duty as a defense attorney. Cady was illiterate at the time and unaware of Bowden's actions. After his release from prison, Cady tracks down Bowden. The former convict learned to read and studied law in prison, and even assumed his own defense, unsuccessfully appealing his conviction several times. Cady hints strongly that he has learned about Bowden burying the report, stating that the judge and prosecutor in his case did their jobs while Bowden betrayed his own client.

In 1973, Sam "Ace" Rothstein (Robert De Niro) is a sports handicapper and Mafia associate who is sent to Las Vegas to run the Teamsters Union-funded Tangiers Casino on behalf of the Chicago Outfit. He hires old friend Billy Sherbert (Don Rickles) as his manager. In between, Ace and his friend, mob enforcer and caporegime Nicholas "Nicky" Santoro (Joe Pesci), narrate how the mob bosses control the Teamsters Union, which gives out money for casinos that they own, such as The Tangiers, and how they also drive off rival crews and get rid of cheaters. Ace becomes the Tangiers' de facto boss by taking advantage of lax gaming laws allowing him to work at the casino while his gaming license is still pending. He doubles the casino's profits, which are skimmed by the Mob before the records are reported to income tax agencies. The bosses are impressed with Ace's work and send Nicky to protect Ace and the whole business, along with Nicky's brother Dominick, Nicky's friend and subordinate Frank Marino (Frank Vincent), and the rest of Nicky's soldiers in his crew. Nicky, however, becomes more of a liability than an asset; his criminal activities- which he makes nearly no effort to conceal- and his violent and vicious temper quickly gets him banned by the gaming board from every casino, and his name is placed in the Black Book. In retaliation, Nicky gathers his own crew, opens a jewelry store and restaurant, begins running unsanctioned shakedowns and burglaries, and soon after is considered the mob boss of Vegas.

Some time thereafter, Jake spots a 15-year-old girl named Vickie (Cathy Moriarty) at an open-air swimming pool in his Bronx neighborhood. He eventually pursues a relationship with her, even though he is already married. In 1943, Jake defeats Sugar Ray Robinson, and has a rematch three weeks later. Despite the fact that Jake dominates Robinson during the bout, the judges surprisingly rule in favor of Robinson and Joey feels Robinson won only because he was enlisting into the US Army the following week. Paul Hackett, a word processor, meets Marcy Franklin in a local cafe in New York. They discuss their common interest in Henry Miller. Marcy leaves Paul her number and informs him that she lives with a sculptor named Kiki Bridges, who makes and sells plaster of Paris paperweights resembling cream cheese bagels. Later in the night, under the pretense of buying a paperweight, Paul visits Marcy, taking a cab to her apartment. On his way to visit Marcy, a $20 bill is blown out the window of the cab, leaving him with only some spare pocket change. The cab driver is furious that he cannot pay, thereby beginning the first in a long series of misadventures for Paul that turn hostile through no fault of his own. At the apartment Paul meets the sculptor Kiki and Marcy, and comes across a collection of photographs and medications which imply that Marcy is severely disfigured from burns on her legs and torso. As a result of this implication, and as a result of a strained conversation with Marcy, Paul abruptly slips out of the apartment.
